.@MiamiVAMC : Imagine your favorite sports team loses every game for like three years straight and the owners finally stepped in, but instead of firing the coach they just give him a new office down the hall. That is exactly how executive accountability works in the @DeptVetAffairs. #VISN8 network is massive Veterans Affairs healthcare system, & covers Florida & parts of the Caribbean & one specific executive David Isaac. Isaac was the person responsible for overseeing operations across the regional network and the explicit claim is that under his direct oversight the network just operated poorly for years, but the Miami VA is where three different directors were forced out but here’s a kicker they were not fired. They were allowed to just quietly retire so basically no real consequences and some Veterans lay the blame for this really toxic low accountability environment directly at the feet of David Isaac & the VISN leadership. But is it really fair to place the blame for localized failures solely on the network. It’s really tempting to blame this size of the bureaucracy. Actually, the tone set at the top the culture trickles down, if a network overseer consistently lets facility directors fail without facing real termination, just letting them take a comfortable retirement instead signals to the entire organization that consequences are basically optional so the argument here is that the localized failures in Miami were a direct inevitable symptom of that permissive oversight at the VISN8 level, which brings us to what actually happened to Isaac himself because if the culture of consequence was nonexistent for his subordinates, what were the consequences for him, he certainly wasn’t fired. He was removed and made a deputy Director and is described as a “mess up move up system” You can never achieve real reform when an administration continually relies on what they call leftover management. It comes down to the underlying mechanics of how bureaucracy protect themselves, firing a top executive is a loud public admission of systemic failure. It invites unwanted scrutiny, it makes a whole agency look bad. But quietly demoting them to a deputy role just sort of sweeps the failure under the rug. The problem is that person is still in the executive sweep. They bring the exact same mindset, same habits, and, the same lack of urgency to the table. It actively prevents structural reform because the old guard is still holding on just from a slightly different chair. The ultimate Takeaway here isn’t just that bad management hurts the end user though obviously it does, it’s that massive bureaucracy are often designed for institutional self preservation not user outcome keeping leftover management around is actually a feature not a bug of a system. Trying to avoid public accountability when survival of a bureaucracy becomes more about lateral moves and avoiding bad PR than actual performance the whole foundation is just Ruff. If rigid bureaucracies inherently protect their under performing leaders through these lateral moves and deputy titles just to save face, can they ever really fix themselves or does real accountability require an entirely external independent oversight board because you simply cannot rely on the front office to fire the head coach if they are all playing for the exact same team

Hey @VetAffairsDems and @RepMarkTakano, put the politics aside for once and focus on helping Veterans. Hundreds of Veterans remain in VA hospitals b/c no one is legally authorized to make medical decisions for them. Now, @DeptVetAffairs attorneys will be able to ask state courts to appoint non-VA employees as guardians for these Veterans. This will give Veterans appropriate legal representation so they can get the most appropriate care in the safest and least restrictive setting possible. Do better.
