The goal of this procedure is to re-establish normal stooling; so someone can sit on the toilet and pass bowel movements like anyone else, and not have to handle the unpleasantness and indignity of living with an ostomy. However, the vast majority of patients living with an ostomy do not suffer from long-term dysfunction or major complications Because UC is a disease that frequently affects young people, the IAPP is not uncommonly performed in adolescents. IAPPs are always elective, and is technically complex requiring multi-stage operations. Rates of short term and long term complications are high and often severe, including development of perianal fistulas, surgical infections, recurrence of inflammation in the pouch requiring long term medical therapies to control. Many patients still struggle to defecate normally due to the effects of this recurrent inflammation, and the fact that this new rectum does not have the same properties as a natal rectum; this can lead to feelings of always having to defecate, incontinence, and bleeding While a malfunctioning IAPP can be reversed, the surgery to do so can also be complex, and the pouch cannot be removed, meaning even patients who have it reversed may have ongoing incurable symptoms Even in a best case scenario, roughly 75% of persons living with an IAPP will say the function is suboptimal, yet on the same hand, over 90% of long term IAPP do not express regret about the procedure, preferring it to having an ostomy, even when pouch function is suboptimal Yet, in spite of this, there is next-to-no discussion about the ethics around consent for young people who wish to have an IAPP. Older adolescents can consent to this procedure if they possess sufficient understanding of the risks and benefits, and adults of younger children can consent on behalf of their children See where I am going with this?
