Contentious thought on a Sunday: Is there any reasons NOT to give neo-adjuvant C(R)Tx prior to resections for locally advanced GI cancer until we have proper genomic/biologic selection markers? I mean "Biology is King" & the time of therapy selects winners quite decently?

One relevant problems in medicine is commercially driven extension of indication. Often you have overlapping groups, one that needs a treatment, one that wants a treatment. Doctors should respond to needs, not invent indications to cater to „wants“. @PrecisionSo @DrUmeshPrabhu
