Sustained transaminase elevation after pancreatoduodenectomy defines a liver perfusion failure phenotype associated with markedly increased morbidity and mortality ➡️ doi.org/10.1093/bjsope… Liver perfusion failure (LPF) following partial or total pancreatoduodenectomy lacks a consensus definition, contributing to systematic underreporting of an ischaemic complication with substantial clinical consequence. This single-centre analysis of 815 consecutive patients resected between 2014 and 2017 tested four candidate definitions and identified an alanine aminotransferase/aspartate aminotransferase elevation ≥ 200 U/l on two consecutive postoperative days (LPF_2d) as the most clinically discriminative model, with an overall LPF rate of 9.9% and the highest area under the curve (0.738) among the models evaluated. Moderate or severe LPF was associated with higher rates of liver-specific complications (40.4% versus 2.9%), postoperative liver failure (30.8% versus 1.6%), major complications (48.1% versus 14.6%) and 90-day mortality (21.2% versus 2.9%). Angiography-guided thyroidectomy was both cost-saving and health-improving compared with conventional thyroidectomy in a Spanish National Health System model. ➡️ doi.org/10.1093/bjsope… Postoperative hypoparathyroidism is the most frequent complication of total thyroidectomy and imposes lifelong monitoring, supplementation, and quality-of-life burden. This model-based economic evaluation compared angiography-guided thyroidectomy, in which indocyanine green fluorescence maps parathyroid arterial supply before dissection, with conventional total thyroidectomy from the perspective of the Spanish National Health System, using a state-transition Markov model over a 35-year horizon with 3.5% annual discounting and a willingness-to-pay threshold of €30 000 per quality-adjusted life-year. Angiography-guided thyroidectomy was associated with lower lifetime costs (€563.7 million versus €613.2 million) and greater health benefit (310 997 versus 299 107 QALYs), yielding 11 889 incremental QALYs and an incremental net monetary benefit of €406.2 million (95% uncertainty interval €381.7 million to €431.5 million). Dominance persisted across deterministic, probabilistic, scenario, and bootstrap analyses, with breakeven activity of approximately 147 thyroidectomies per centre per year. Impact of thromboprophylaxis timing and duration on thromboembolism and bleeding in metabolic bariatric surgery ➡️doi.org/10.1093/bjsope… Optimal timing and duration of thromboprophylaxis in metabolic bariatric surgery remain uncertain despite guideline recommendations based largely on expert consensus. This nationwide registry-based cohort study used data from the Scandinavian Obesity Surgery Registry, including 83,801 primary bariatric procedures performed in Sweden between 2008 and 2023, to evaluate associations between thromboprophylaxis timing and duration and the risk of venous thromboembolism (VTE) and bleeding. The overall incidence of VTE was low (0.1%), while intraoperative and postoperative bleeding occurred in 0.7% and 1.6% of patients, respectively. Postoperative initiation of thromboprophylaxis was associated with a significantly lower risk of intraoperative bleeding compared with preoperative initiation (odds ratio 0.52; 95% confidence interval 0.37–0.74), with no corresponding increase in VTE risk. Neither the timing nor the duration of thromboprophylaxis was significantly associated with VTE or postoperative bleeding. 🏥 Scheduled intravenous acetaminophen versus thoracic epidural analgesia for postoperative pain control after minimally invasive gastrectomy: multicentre randomized non-inferiority trial 📜 Some 140 patients were included, and patients receiving intravenous acetaminophen had comparable pain scores when compared to thoracic epidural analgesia (risk difference -4.1 per cent; 95 per cent confidence interval -24.3 to 16.6). Hospital-level structural, staffing, and system characteristics are associated with variation in failure to rescue after surgery. ➡️ doi.org/10.1093/bjsope… Failure to rescue (FTR), defined as death following a postoperative complication, is a key driver of between-hospital variation in surgical mortality, yet the institutional characteristics underlying this variation remain inconsistently described. This systematic review and meta-analysis synthesised 111 observational studies encompassing more than 137 million surgical patients and 301 hospital-level factors across structural, staffing, and system domains. Lower FTR was associated with larger hospitals (OR 0.85, 95% c.i. 0.74 to 0.97), greater intensive care unit capacity (OR 0.80, 0.71 to 0.90), and teaching status (OR 0.88, 0.83 to 0.93), while nursing resources showed the most consistent associations, including patient-to-nurse ratios (OR 1.07, 1.03 to 1.10) and more favourable work environments (OR 0.93, 0.90 to 0.95). Safety-net hospital status was associated with higher FTR (OR 1.22, 1.03 to 1.45), with substantial heterogeneity across analyses. A Bayesian, electronic medical record–derived algorithm enables near-real-time monitoring of postoperative complication rates after major colorectal surgery ➡️doi.org/10.1093/bjsope… Conventional surgical quality assurance programmes, including large national registries, are resource-intensive and rely on frequentist methods that report adverse events months after they occur, limiting their capacity to inform timely intervention. This retrospective cohort study developed an artificial intelligence–assisted algorithm to extract surgical outcomes directly from the electronic medical record and applied Bayesian binomial models with non-informative priors to estimate the posterior probability of complications among 747 patients who underwent major colorectal operations at a single academic health system between 2016 and 2024. Postoperative ileus (7.4%) and anastomotic leak (3.9%) were the most frequent complications, whereas postoperative bleeding was rare (0.3%); across the ten tracked complications, the number of events triggering chart review ranged from two in 2016 to six in 2022, with no more than three excess complications in any single year. Applying an 80% posterior probability threshold for exceeding institution-derived benchmarks produced a manageable volume of cases for review, whereas a conventional 95% threshold flagged almost none, illustrating the limitations of fixed-α rules under repeated monitoring. Multi-outcome prognostic modelling for older adults after trauma: development and validation of the Older Trauma Outcome Predictor (OTOP) model ➡️doi.org/10.1093/bjsope… This study developed and validated the Older Trauma Outcome Prediction (OTOP) model, the first prognostic tool designed to predict multiple inpatient outcomes in older trauma patients. Using data from more than 105 000 patients in the Western Australia Trauma Registry, OTOP accurately predicted mortality, surgical intervention, intensive care unit (ICU) admission, length of hospital stay, medical complications, and discharge destination. The model demonstrated excellent performance for mortality, surgery, and ICU admission and outperformed existing mortality-focused tools. Hospital costs associated with complications following pancreatoduodenectomy: systematic review ➡️doi.org/10.1093/bjsope… This systematic review, including 46 studies encompassing 111 215 patients from 13 countries, quantified the hospital costs of complications following pancreatoduodenectomy, with all figures adjusted for inflation and purchasing power parity to 2023 Dutch euros. Clinically relevant complications imposed substantial financial burdens, with grade B postoperative pancreatic fistula adding €1998–€25 228 and grade C up to €112 824, while delayed gastric emptying, postpancreatectomy haemorrhage, biliary leak, and higher Clavien–Dindo grades similarly escalated costs.
