Select the cardiac preset on the device, if available. Palpate the apical impulse and place the probe at this location. This is approximately at the anterior axillary line along the inframammary fold in female patients or below the nipple in male patients. Obtain the apical four-chamber view, and then tilt the beam anteriorly and cephalad to include the LVOT. If the view is limited, consider repositioning the patient or placing the probe in a different intercostal space. Other image optimization tips include using more ultrasound gel, applying more pressure with the probe, and instructing the patient to hold their breath in exhalation. Once an adequate view is obtained, select the PWD button on your device. Place the PWD gate in the center of the LVOT, just proximal to the aortic valve. The PWD gate should be parallel to the direction of the blood flow in the LVOT. Care should be taken to ensure the PWD gate be no more than 20 degrees from the LVOT axis. Once the PWD sampling gate is in the proper position, the PWD button can be activated and a velocity waveform will be visible on the screen. The waveform will include both positive and negative signals. Adjust the baseline up or down so that the negative deflection of the signal is fully captured on the screen. Next, select VTI from the device's measurement button and choose the manual option. Starting from the baseline at the left-most part of the negative deflection, trace inferiorly along the outermost edge of the waveform, extending around the trough, and return to the baseline at the right-most part of the waveform. If there is beat-to-beat waveform variability, an average of three to five consecutive VTI measurements can be calculated.
