Impact of glycaemic control on complication rates after breast reconstruction ➡️doi.org/10.1093/bjsope… Breast reconstruction remains integral to breast cancer care, yet optimizing outcomes in patients with diabetes is challenging, particularly regarding perioperative glycaemic control. In this NSQIP-based retrospective cohort study (2021–2023), preoperative HbA1c was not associated with increased 30-day complications in implant-based reconstruction, but was significantly associated with higher morbidity in autologous reconstruction. Patients with poor glycaemic control undergoing autologous procedures experienced markedly higher complication rates, likely reflecting the greater physiological stress and operative complexity of these surgeries. Perioperative management of respiratory conditions and complications ➡️ doi.org/10.1093/bjs/zn… 🌬️ Postoperative pulmonary complications (PPC) occur in 1–23% of major surgical patients and remain a leading cause of morbidity, ICU use, and prolonged stay. 🚩 Risk rises with age, smoking, COPD/asthma, recent respiratory infection (<1 month), obesity, OSA, and high-risk surgery (thoracic/upper abdominal/emergency). 🩺 Tools like ARISCAT, baseline SpO₂, and functional testing (6MWT/CPET) help identify high-risk patients early. 🛠️ Optimisation works: stabilise lung disease, stop smoking (>4 weeks ↓ risk), treat infection, use lung-protective ventilation, opioid-sparing analgesia, and early mobilisation. 🎯 PPC is often preventable with structured perioperative care. Twelve experts rated decision-making and situation awareness using NOTSS. 📊 Results: Mean cognitive NOTSS 5.6/8, with strong correlation between domains (r = 0.8; P < 0.001).
