🚨 PHOTON COUNTING CT IS BECOMING AN ALTERNATIVE TO CARDIAC MR🚨 🆕 Photon-Counting CT (PCCT) is changing the game in myocardial tissue characterization! 🫀💥 Tremamunno et al. Eur Rad 2025. doi.org/10.1007/s00330… ✨ With iodine maps and ECV quantification, PCCT can now detect late enhancement (LE) with high accuracy — rivaling the gold standard Cardiac MRI (LGE)! 💯 🔍 Why it matters: ✅ High diagnostic accuracy ✅ Detects both ischemic & non-ischemic fibrosis ✅ Fast, accessible, and MRI-free ✅ Ideal for patients with implants or claustrophobia ✅ Real One-stop-shop for coronary + myocardial assessment 🛠️🫁 🧠 Summary with Key Highlights 📘 This study by Tremamunno et al. investigated whether iodine maps derived from photon-counting detector CT (PCD-CT) can accurately detect myocardial late enhancement (LE) — a key marker of myocardial fibrosis or injury — compared to the gold standard late gadolinium enhancement (LGE) on cardiac MRI. 🏥 Study Design 27 patients (52% female, average age 53) underwent both cardiac MRI and PCD-CT. PCD-CT was performed using a delayed-phase protocol with iodine-based contrast (late iodine enhancement or LIE). Iodine maps were generated using spectral capabilities of PCD-CT to mimic the nulling effect of MRI and highlight fibrotic tissue. 🔍 Key Results Per-patient diagnostic accuracy of PCD-CT iodine maps: 85.2%, with 100% sensitivity (reader 1) and 91.7% sensitivity (reader 2) 💯. Per-segment accuracy: >90% with high specificity (>94%) but slightly lower sensitivity (~67–75%) for subtle or non-ischemic fibrosis. Image quality was consistently rated good to excellent, with strong inter-reader agreement (κ = 0.70 per-patient; κ = 0.63 per-segment) 🤝. Best performance was in detecting ischemic scars, especially transmural and subendocardial patterns. Non-ischemic fibrosis, particularly mid-wall and patchy types, was more difficult to detect. 💡 Why It Matters PCD-CT now offers a clinically viable alternative to LGE-MRI for myocardial tissue imaging 🎯. Especially valuable for: Patients with contraindications to MRI (e.g., pacemakers, claustrophobia). Patients not able to withstand lengthy CMR examinations. Time-sensitive scenarios requiring rapid assessment. One-stop-shop protocols evaluating both coronary arteries and myocardial viability 💡🫀. ⚠️ Limitations Small sample size (n=27). MRI still superior in detecting subtle non-ischemic changes due to better soft tissue contrast. ✅ Conclusion 🔬 While MRI remains top-tier for soft tissue contrast, PCCT is no longer the future — it’s the present. 🔄 #CardiacImaging #PCCT #PhotonCountingCT #CardioCT #MRIalternative #LateEnhancement #ECV #Radiology #HeartHealth ❤️🧠🖥️
