Congrats to the @AgborOmicsLab for their insightful work using cf-DNA to assess the recipient inflammatory milieu and use that to prognosticate post-txp outcomes. I’m honored to write the accompanying editorial.

Congratulations to @KaitlynChapinNP on her initial first-author paper. We examined risk factors for and outcomes of extubation failure in lung txp recipients.

@VUMCTransplant Thanks for highlighting this nice work led by @johnwstokes @whitney_gannon and @MatthewBacchet1. Glad I could be part of the “et. al” : ) This work justifies the use of ECMO in lung txp recipients outside the BTT and PGD realms.

How to carefully select patients for VV #ECMO? #ECLS is not for all, resources are limited; to be balanced:
⚖️ costs staffing ICU beds
⚖️ patient survival
Goal social justice, utility, respect for the person.
ECMO mostly aimed as bridge to recovery: Who will cross the bridge? How about the quality of the bridge
How to predict success?? Prediction tools provide a structured algorithm using objective criteria to prognosticate success
➡️ensures uniformity in pt selection
➡️minimizes inherent biases
➡️establishes expectations with care team, pt, family

.@AnilTrindadeMD et al. demonstrate that change in dd-cfDNA% is a more accurate indicator of ALAD than an absolute threshold in a cohort of lung transplant recipients >2 y posttransplant. #OpenAccess #VisualAbstract #TransplantTwitter bit.ly/3PpjP43
