PBMs paying influencers $13,000 for a single video that’s coordinated to protect the PBMs vertical integration model - aka monopoly. This year, TN is working hard to break-up pharmacy monopolies, increase competition, improve access and protect the free-market. We remember those letters well. The ones that arrived right after CVS Caremark slashed our reimbursement rates to somewhere between “thank you for your service” and “please don’t make us pay the electric bill.” The ones that expressed such deep, heartfelt concern about the “current economic pressures facing independent pharmacies.” The ones that somehow always ended with a polite invitation to sell out before we, you know, went broke. We were touched. Really. So now, in the same spirit of neighborly compassion, we at Johnson’s Corner Pharmacy (one scrappy independent, zero PBMs, still somehow turning a profit) would like to return the favor. We are prepared to make a **very generous offer** to purchase all 134 of your Tennessee CVS pharmacies—lock, stock, red boxes, and those little “next window please” signs. Our valuation model is simple and transparent: we’ll pay you exactly what CVS Caremark has been reimbursing us for the past few years. You know the rate—the one that makes a grown pharmacist cry into their compounding hood. It’s only fair. After all, turnabout is pharmacy justice. Think of the benefits this acquisition will bring you: - You’ll finally be free to focus on what CVS does best—lobbying Congress, perfecting the 47-step app checkout, and reminding patients that “health is everything” while your mail-order division quietly undercuts the very stores you own. - We’ll take over the leases, the MinuteClinics, the endless corporate KPIs, and the soul-crushing drive-thru metrics. - Your shareholders won’t have to watch another earnings call where someone asks why the retail division keeps getting squeezed by… well… you. Under independent ownership, these stores could become actual community pharmacies again—places where patients are greeted by name instead of by their insurance BIN number. Revolutionary, we know. If you have a deal with the biggest PBMs you are getting ripped off A simple way to realize this is to take both your insurance and PBM contracts and out them in @claudeai and just ask the following "How am I getting ripped off and what can I do about it " If you do, please post how it responds here so everyone can learn !

I applaud @SenTomCotton for standing up for uniformed service members, retirees, and their families by ensuring that they have access to prescription drugs at pharmacies of their choice. The Rx ACCESS Act, which Senator Cotton has co-sponsored, would require pharmacy benefit managers (PBMs) to reimburse pharmacies participating in the TRICARE program either the actual cost of a drug or the national average drug acquisition cost, thus shielding independent pharmacies from unfair reimbursement practices and empowering military members and their families to use the pharmacy they choose—not the one chosen for them by the PBMs. As an almost 30-year Army officer, my family and I have seen firsthand the anti-competitive ways PBMs manipulate the market and burden servicemembers. For far too long, PBMs have abused their power to the detriment of consumers and independent pharmacies to benefit themselves. This Sounds Like “Stark Derangement Syndrome” Hospital systems employing physicians with mandates to self refer internally, penalizing "leakage," and owning every ancillary service. Same incentives @FAHhospitals decries yet on a monopoly scale that drives prices up prices to patients 10-40% with no quality gain. Physician owned hospitals deliver 8-15% lower Medicare costs for identical patients, equal/better quality, and real competition. 86+ groups back repeal via H.R. 4002. There is no issue with physician-led hospitals- the issue is about the conflict of interest when physicians self-refer patients to their own hospitals. The data is clear: POHs tend to treat more commercially insured and healthier patients than full-service hospitals. In rural communities, this can leave rural hospitals with a greater financial burden, further threatening their ability to keep their doors open and keep 24/7 care available in their communities.
