Sinus bradycardia is a typical feature, but of course the initiation and dosing of beta blockers will impact incidence of pacing indication and probably has dropped since we have flecainide.
Can RYR2 cause DCM, how strong is the evidence? I have an infant with this mutation and DCM (however was transient and likely viral?). No FH of note. Will do segregation analysis and holter, etc. i did not see the ecg yet. @ChrisvanderWerf@jodieingles27@jpkaski@jeffrey_vinocur
@Mo_PedsEP@seshadribalaji6 Not sure about the additional value of an ILR in an asymptomatic patient, but fully agree with very close noninvasive monitoring and considering LCSD before ICD in case of ventricular ectopy or possible nonadherence
@seshadribalaji6@ChrisvanderWerf I would at least do loop implant, consider sympathectomy (if that is easy enough) and repeat EST frequently and make sure of compliance
Lost one of my TECRL CPVT pts despite being dx at 14 yrs old bcz of cascade screening (asymptomatic) but with a +ve EST that normalized on nadolol/flecainide (compliant). While my practice is no ICD in asymp & have no breakthrough with tx. Not sure now with atypical CPVT forms?
