NEW | Evening Iran Update 🧵(1/3): US naval forces continued to enforce the blockade on Iranian ports. US Central Command (CENTCOM) reported on April 15 that no vessels had breached the blockade on Iranian ports during the first 48 hours of enforcement. CENTCOM stated that nine vessels complied with the US direction to turn around and return to Iranian ports or coastal areas. CENTCOM added that US forces have halted economic maritime trade to and from Iran and are prepared to act against any vessels attempting to violate the blockade. Additional CENTCOM reporting indicates that the US forces redirected at least one Iranian-flagged cargo vessel after it departed Bandar Abbas, Hormozgan Province. 2. The United States has reportedly set two preconditions for another round of negotiations: first, Iran must fully “reopen” the Strait of Hormuz, and second, the Iranian negotiating delegation must have “full authority” to finalize a deal. The second precondition comes amid reports of intra-regime conflict and corroborates ISW-CTP’s assessment that the United States is negotiating with a divided council of hardliners and pragmatists rather than a singular, unified authority. 3. Some Iranian officials have signaled different levels of willingness to compromise on Iran’s nuclear program in negotiations with the United States. These statements, at least to some extent, reflect internal disagreement over Iran’s negotiating position. 4. Israeli Defense Minister Israel Katz stated on April 14 that the United States and Israel have decided that removing the enriched uranium from Iran is a “threshold condition” for ending the US-Israeli campaign. 5. The US Treasury sanctioned 29 targets, including three individuals, 17 companies, and nine vessels on April 15, which are tied to Iranian oil smuggling and associated financing networks as part of the United States’ broader economic pressure campaign on Iran. 6. Pakistan is reportedly pushing the United States and Iran to extend the ceasefire by 45 days. US President Donald Trump told ABC News on April 14 that he is not considering extending the ceasefire that ends on April 22. 7. Iran is exploiting the current ceasefire to reorganize and regenerate its ballistic missile forces program on the tactical level by attempting to reopen tunnel entrances at missile bases. 8. Iran reportedly utilized an advanced reconnaissance satellite purchased from the People’s Republic of China (PRC) in 2024 to target US military assets and bases across the Middle East during the war. 9. The Israeli Security Cabinet reportedly held a meeting on April 15 to discuss a possible ceasefire in Lebanon, amid recent direct talks between Israel and Lebanon under US auspices. Israeli officials have not announced the outcome of the security cabinet meeting at the time of this writing, however.

NEW: Indirect US-Iran negotiations continue. The United States sent a draft proposal to mediators on April 14, and the Pakistani chief of defense forces arrived in Tehran on April 15, according to Axios. It does not appear that either side has publicly compromised on their red lines, however. Israeli Defense Minister Israel Katz stated that removing highly enriched uranium from Iran is a “threshold condition” for the end of Israel’s military campaign. Katz implied that Israel had coordinated with the United States over this requirement. US Central Command Admiral Brad Cooper announced that US forces continue to implement the naval blockade against Iranian ports and have completely halted economic trade to and from Iran. One of the two reported decisionmakers in the Islamic Revolutionary Guards Corps (IRGC) leadership called the US blockade a “prelude to a ceasefire violation,” adding that Iran will continue its blockade of the Strait of Hormuz. The current ceasefire allows Iran’s missile force to reorganize itself and prepare for a new round of fighting, should the war resume. The operational-level disruption of Iran’s missile force, particularly its medium-range force, was temporary and can only be maintained through military pressure. @BenTzionMacales identified Iranian engineering equipment at Tabriz South missile base in East Azerbaijan Province and the Khomein missile base in Markazi Province, both of which have been struck several times in the US-Iran conflict. The combined force disrupted efforts to dig out missiles before the ceasefire by striking this equipment.

Definitely do not stop statins, and preventative LDL-cholesterole lowering therapies in general, based on these arguments, as they clearly misrepresent the facts. The AMORIS study only showed that extremely low total cholesterol was negatively associated with the chance of becoming a centenarian (reaching 100 years of age) (observational association; not causal, and low total cholesterol can also reflect underlying illness in older adults). Total cholesterol is different from LDL. Statins specifically lower LDL in the blood by increasing LDL receptor expression in the liver. The points made here perpetuate a common misconception, often propagated by health influencers, which is mixing up the beneficial functions of a molecule (nearly all naturally occurring biological molecules have legitimate functions in cells, tissues, and organs) with the level of these molecules in the blood, abnormally high levels do not mean a particular molecule is doing its job better, for a simple example, think of glucose, it fuels the brain, too much would not cause your brain to function more, the contrary as it causes metabolic disease, diabetes, and even increases cognitive decline risk. Cholesterol, and even LDL, have their legitimate functions, but these functions do not improve when the molecules are abnormally high in the blood. In fact, elevated LDL in the blood is often an outcome of defective liver uptake, frequently due to metabolic disease, and in rarer cases, solely due to genetics (familial hypercholesterolemia). LDL is causally linked to cardiovascular disease, and lowering it reduces heart attack risks in patients with existing plaque. Mendelian randomization studies also show that lifelong exposure to lower LDL levels reduces coronary artery disease risk; a 38.7 mg/dL reduction correlates to over a 55% reduction in coronary artery disease risk. Moreover, loss-of-function mutations in PCSK9 lead to a 28% reduction in LDL and correlate to an 88% reduction in coronary heart disease. The genetic variants with lower cholesterol share a similar mechanism with statin or PCSK9 treatments, in that they mainly lower LDL by increasing liver clearance. The U-shaped relationship between LDL-cholesterol levels and all-cause mortality at a population level is likely driven by reverse rather than actual causation, since many aging and chronic disease drivers, including inflammation, malnutrition, cancer, and liver disease, can all contribute to lowered levels of lipoprotein. Contrary to the observational data, a large meta-analysis showed that more intensive statin regimens and further reductions in LDL cholesterol produced further reductions in heart disease events and death risk. 20% reduction in coronary heart disease death and 10% reduction in all-cause mortality per 1 mmol/L (38.7 mg/dL) LDL-C reduction across trials. With that said, you might want to reconsider statins in the following cases: A severely lowered LDL coupled with partially increased blood glucose can be a sign of cholesterol "overmedication." The same mechanism that leads to increased clearance by liver uptake also works to increase LDL uptake by pancreatic islet cells. Preclinical evidence shows that overmedication to lower blood LDL can lead to disrupted pancreatic islet functions and insulin secretion, and increased blood glucose (In humans, statins are associated with a modest increase in diabetes risk overall; this is not reliably diagnosed by “LDL too low,” and risk is most evident in predisposed individuals). Consider swapping to PCSK9 inhibitors. This is a well-established injectable (once every month or every two weeks) class of biologics (recombinant monoclonal antibodies targeting PCSK9 and preventing its effect in degrading the LDL receptors in the liver). PCSK9 inhibitors have shown added protective effects in lowering cardiovascular disease risk even in patients already on statins. They also have a speculative anti-aging mechanism since circulating PCSK9 has been shown to increase with aging. While they can also plausibly interfere with the pancreatic islets and cause similar disruption seen with statins, there is no evidence yet to date of PCSK9 inhibitors increasing diabetes risk, such as evidence exists for statins (major outcomes trials have not shown a significant increase in new-onset diabetes over ~2–3 years of follow-up). If your HDL is too low, you need to pursue strategies to increase it, including exercise, healthy diets rich in MUFA and Omega-3 fats and fibers, avoiding added sugar and refined carbs, and abstaining (or seriously cutting down) on alcohol. If you are experiencing muscle ache and discomfort with statins, this is also a strong reason to switch. While the more acute form of muscle degeneration is very rare and would have been detected earlier as you began your statin therapy, lower-grade muscle aches are common in statin users. Changing to PCSK9 inhibitors, or other medications like ezetimibe (a cholesterol absorption blocker), can also help. Personally, I am currently on Repatha, a PCSK9 inhibitor, as I replaced statins after noticing they slightly increased my blood glucose.

UNSOLICITED ADVICE TO TYRO-FILIPINO-LAWYERS INTENDING TO APPEAR BEFORE INTERNATIONAL PROCEEDINGS. I have been appearing before international arbitrators in a number of commercial arbitrations abroad since the late 1990s and I can say that what we saw last night at the Hague is not far from what is being done in international arbitrations. And so, my unsolicited advice to Filipino lawyers without experience appearing in international tribunals are the following: 1.) There is a decorum of formality in the room. Unlike in local courts where, before the hearing, opposing counsel can talk, joke and even appear to be happy with each other, in international litigation/arbitration, once you enter the court/arbitration room, the impression that you are professional adversaries even before the start of the hearing becomes clear. No talking to each other, no casual or cavalier greetings. There was a time when my opposition was a retired Philippine magistrate who was a novice in international arbitration. Because we know each other, he attempted to approach me before the hearing in the litigation room. I just smiled and gestured to stop approaching me. After the hearings, I had to talk to him and just explained that what I did was part of the unwritten decorum required. He understood. He learned. 2.) International Judges/Arbitratrors are very strict on schedules. This is very important and , unless there is truly a very compelling reason, there will be no postponements or reschedulings. That is why , in this Duterte Case, should the charges be confirmed and trial proceed, whoever will be the Filipino counsel of Duterte will most likely experience continuous trials for an average of two-weeks per setting. That is half a month per scheduled setting. So be prepared. 3.) Unlike in local courts where you can make manifestations after manifestations, nothing of that sort in international arbitration/litigation. The reason is that all concerns must already be ventilated prior to the hearings. These concerns will be resolved posthaste so that the schedule will not be disrupted. 4.) You always have to be respectful. If you do not like the forum or you wish to inhibit an arbitrator, you should do it prior to any hearing. They will stick to the schedule. 5.) Study your case very very very well. I have seen a situation when an opposing counsel asked the arbitrators that he wanted a little bit of time to go over his files, the counsel was told that it was just a waste of the arbitration's time and so , the arbitrators moved to another point. 6.) The standard is very high. In oral presentations or summations , you may even be asked about the footnotes in your submission. When I appeared in international hearings, I had all the summaries of cases ( including the footnotes) in my index cards and then in my laptop. And I had to review all of them, together with copies of the exhibits, the night before the hearing. I have retired from active practice except in international arbitration. I learned to like the discipline involved and , yes, the scheduling.
