Going to have a rant. 82yo lady post hip fracture. No HF symptoms but someone does a BNP. 11000. Echo EF 19%. Cardio reg + HF nurse spiro, dapagliflozin and sorry while I compose myself ENTRESTO. Pt euvolaemic + asymptomatic. Read your own guidelines guys. I’m stopping them all.

What can we learn? That dying is inevitable, recognisable, describable, and that we can prepare for it. The Queen had clearly planned ahead. That at the edge of life, we can still enjoy love, and peace, and companions. That we need to get familiar with dying. 12/12

My hospital has one system for Obs, bloods, radiology, prescribing etc. My housemate has moved to a trust that uses SEVEN different systems for routine usage. Why? Why are things like this? These hospitals are 30 miles away, but about 20 years apart
