In the paper, we cite sources demonstrating that male adults in their 30s with T1D have positive CAC scores 30-70% of the time, suggesting a basis for not anticipating a score of 0 in this age group. That said, I agree the helpful health behaviors likely helped.

LDL: guidelines suggest targets of <70 or <100. Regardless, this value was above normal (60th percentile) for age and sex matched adults with T1D.

CT calcium scores only detect the end stages of plaque development, calcified plaque, but given his plant based diet, and young age, I would not even expect to see much soft plaque.

This represents a black sheep phenomenon for the traditional LDL and CAD link. This one case report (of over 2.4 million single patient publications) adds nuance to the medical research literature, provoking new thinking.

Carotid artery IMT was not assessed in this patient, thus it remains unknown whether a CTA would have been indicated (e.g., if IMT was >1.5mm).
