@brianchiong Thoughts on using a flow switch on your micropuncture sheath and around your IJ wire when creating your tunnel for tunneled central venous access? I had never seen it done residency and fellowship but a few people in my new group use it regularly.

So a little air in the line going to the peripheral IV, probably not a big deal. When to worry? Patients with central lines. Don't leave them uncapped, deep breath could pull in enough air. Same thing when placing and removing. 17/

@ShofnerWhitney My sheath flip technique: Replace sheath with a reverse curve catheter and pull back until glidewire selects opposite limb of graft, advance wire to straighten catheter, remove catheter and put your sheath back over the wire. (Don’t mind my hands in the field 😆)

First time flipping the sheath on a graft declot! I am curious to know... how do you guys/gals do it? Dual sheaths easily occluded this 4/7mm tapered graft making the declot a nightmare. Finally got it open with a single sheath. Presistence pays off!

65 yo w/pancreatic ca and progressive abdominal pain found to have a severe stenosis at the portal confluence. Stenosis stented via transjugular access w/ significant reduction in pain post-procedure. #IRad #PatientCentered @SIRRFS @SIR_ECS @JVIRmedia @MFickesMD @ncdoro
