This is a fetus so foramen Ovale must be patent. In adults, can do a bubble study if you suspect intracardiac shunt - appearance of microbubbles within 3-6 cardiac cycles after right atrial opacification is considered positive for the presence of an intracardiac shunt. (air is white on US = acts as contrast). But bubble study is generally beyond the scope of #POCUS (depends on ur specialty). Large shunts can be seen on color Doppler as well.

The results from our DEVICE trial are out in the @NEJM . In this RCT, direct laryngoscopy was inferior to video laryngoscopy for first-pass success. In fact, it was so inferior that we had to stop the trial early after DSMB review due to the clear benefit of VL. This is the 🔨 nail in the ⚰️ for DL. While it's an important skill, VL should be the standard when it's available.
