What would you do… patient from OSH with previous fem below knee pop bypass that had iliac stents (now occluded), chronically occluded CFA, outside providers stented bypass and then PT. Now occluded. No AT. Previous SFA stent that’s failed. Rest pain/tissue loss. 😬

Well, The BEVAR looks great but something went wrong below! Tertiary referral for a late seventies patient lost from follow-up after BEVAR in EVAR, dramatic evolution of infrarenal sac, suspected type 2 EL. EVAR 2014 (92mm), BEVAR 2019 (102mm), now AAA diameter is 170mm #aortaEd

@XavierBerardMD @agustinmoscosoi Great case as always Xavier. What would the plan be if samples would have come back positive? Lets say it was an EVAR and not BEVAR, would you have done an explant just to be sure not to leave prosthetic material behind?

@agustinmoscosoi It’s the liquid part of the intra sac thrombus, the sac is still pressurised by the endoleak (better capture in this excerpt) The infection is always a concern even if like in this case nothing is suspicious. Systematic Multiple samples must be done for this purpose #aortaEd

Starting 2025 as we ended 2024, with some redo aortic surgeries. Duodenal Fistula (repaired by direct suture, just done before this picture) on a side-to-end anastomosis. One clamp above, the other below the renal arteries, ready to explant a graft with a greenish spot #aortaEd

@farkomd Frank There is still some distal flow. It is very damaged and the kidney is smaller. But I do think is reasonable to give a try. You may be able to reopen. The cases I give up are a. Complete absence of any distal flow reconstitution and b.very small kidneys (<7cm or so). Best G

4 vessel FEVAR. 2 years out. Left flank pain for 8 hours that persists. Slight increase of creatinine 1.4 from 1.2. Tried to capture pertinent info on CTA. Size of both kidneys measured on CT. Intervene or Leave alone? #AortaEd
