PHOTON COUNTING "DEFEATS" CORONARY STENTS
Coronary stents are everywhere.
Tiny metal scaffolds that save lives, relieve symptoms, and silently sit inside millions of patients.
And yet… when it comes to seeing through them, we’ve been struggling for years.
Conventional CT (EiD) does a decent job with native coronary arteries.
Until reality gets messy.
- Calcifications.
- Metal.
- Blooming.
- Artifacts.
That’s where clarity fades—and uncertainty begins.
And uncertainty is not neutral.
It leads to overcalls, undercalls, unnecessary invasive angiographies… or worse, missed disease.
Now something has changed.
Photon Counting CT doesn’t just “improve” imaging.
It changes the rules.
- Higher spatial resolution.
- True spectral information.
- Less blooming.
- More truth.
Stents stop being blind spots.
They become readable.
For the first time, non-invasive follow-up of stented patients starts to make real clinical sense.
Accurate, reliable, trustworthy.
And once you see it, it’s hard to go back.

And we’re off!
First HeartFlow NAVIGATE-PCI case.
Precision in PCI isn’t just by IVUS/OCT anymore— the standard has expanded to CT-Guided PCI. NAVIGATE-PCI will test how data-driven tools can optimize decision-making, streamline workflow, and improve outcomes in the cath lab.
Optimal PCI is guided, precise, data-driven inside #ctguidedpci or outside #imagefirst the lab

🧰⚙️🔧TAVI via axillary access: a simple, effective approach
🔗pcronline.com/Cases-resource…
When transfemoral access is not an option, the axillary route offers a fully percutaneous alternative for TAVI.
This step-by-step approach shows how to secure access and achieve reliable haemostasis using a combined suture- and plug-based strategy.

This is a fascinating. I, naively, assumed all pts on thyroid replacement needed it. Gosh, 1 in 4 pts could stop the med
I bet if you used a proper placebo the discontinuation rate would be even higher
I also wonder how many other meds such a process would be feasible for
JAMA@JAMA_current
In adults ≥60 years, 26% discontinued levothyroxine while maintaining thyroid function, with 64% success for doses ≤50 μg/d, and no clinically important changes in thyroid-related symptoms. 🧵 ja.ma/41TS48I

Among patients with atherosclerotic cardiovascular disease, targeting an LDL cholesterol level below 55 mg per deciliter led to a lower 3-year risk of cardiovascular events than targeting a level below 70 mg per deciliter. Full Ez-PAVE trial results: nejm.org/doi/full/10.10…

@DrAditya2935 It’s not magic. It’s still long way to go before we can be sure about its safe long term use. Jogging and running are the best.

The last paragraph of this editorial is the first thing we teach our interventional fellows.
“In general, the benefit from any percutaneous intervention in cardiology is often the greatest with regard to the most acute or life-threatening problem the patient is facing. In contrast, in clinical situations in which the patient’s life is not directly at risk and the symptoms are controllable by medical treatment, conservative management should always be considered to be an equally effective alternative.”
