@mpetersenmdphd Agree and HbA1c was adjusted for but HOMA IR not available. One could argue it’s about identifying a higher risk vs benign phenotype. Moreover the time factor of people transitioning to IR

@ProfKausikRay The challenge in MHO research is always the definition. Here, "Participants were considered as metabolically unhealthy if ≥1 of the following were present: hypertension, dyslipidaemia or diabetes". So many "MHO" probably had prediabetes/insulin resistance.

@SABOURETCardio And that is exactly why I chose to study the concept by looking at the independent and joint effects. If you read the discussion you will see why those studies were flawed

@ProfKausikRay Great work Kosh as usual. I am still not convinced since the beginning by the concept of “metabolically healthy “ obesity. Your data confirm that we have to act as early as possible. ☀️

Finally CRP or inflammation was the biggest mediation of risk for all outcomes especially in women. Findings which mirror observations with GLP1s

This would have the practical result of more accurately predicting risk in individuals who may be below treatment thresholds currently for initiating preventive therapies especially women.

The @nationallipid has published an important new meta-analysis evaluating the impact of low-density lipoprotein cholesterol (LDL-C) lowering on major adverse cardiovascular events (MACE) in primary prevention populations. The study synthesizes data from randomized trials of both statin and non-statin therapies and updates the landmark 2012 analysis from the Cholesterol Treatment Trialists’ Collaboration. Prof. @ProfKausikRay said: "I was honoured to collaborate with NLA colleagues on this important research. This systematic review and meta-analysis of all available trial data in patients without pre-existing cardiovascular disease demonstrates that, irrespective of how LDL cholesterol is lowered, there are greater relative benefits than expected compared with starting treatment in patients with pre-existing disease. The findings are immediately actionable and should be used by clinicians in shared decision-making. This greater ‘return on investment’ per 1 mmol/L reduction suggests that health systems should prioritise primary prevention earlier as a means to reduce the global burden of cardiovascular disease." New NLA Publication📢 The NLA’s latest expert meta-analysis strengthens the evidence supporting LDL-C lowering in primary prevention. The analysis demonstrates that LDL-C reduction is strongly associated with lower risk of major adverse cardiovascular events (MACE), strengthening the rationale for early, proactive lipid management before the first cardiovascular event. 📃Read the full analysis: ow.ly/JrAX50YoIxu 🗞️Read the press release: ow.ly/POsT50YoIxt @DineshKalra @ProfKausikRay @ArchnaBajajMD @carolkirkpat3

Our analysis from Swedeheart at changes in TG (up, down no change) over 1 year post MI & risk of events. They go the way you think but trials will need to much larger with more potent therapies and much higher TG online at EJPC doi.org/10.1093/eurjpc…
