NOTION-2 did not demonstrate the noninferiority of TAVI compared to surgery for the primary endpoint at 1 year. Due to fewer events than anticipated in the surgery group, the trial lost statistical power, resulting in widened confidence intervals that crossed both the noninferiority boundary and the line of identity. This is the methodological situation where a noninferiority trial is termed "Inconclusive" because no result can be significantly excluded. We cannot say TAVI is noninferior, we can't say it is inferior, we can't say it is superior. Check mate. That said, NOTION-2 remains valuable for descriptive purposes and inclusion in meta-analyses. Long-term results with increased event rates will provide additional insights on the comparison of TAVI and SAVR in younger, low-risk patients. academic.oup.com/eurheartj/adva…

We evaluated 820 patients treated with TS MViR using the Sapien 3/Ultra/RESILIA THVs between Aug 2015 and Dec 2022 who were enrolled in the STS/ACC TVT Registry. We found a significant reduction in MR and sustained improvement in symptoms at 1 year. Mean mitral gradients were elevated at 30 days (7.9 ± 3.0 mmHg) and at 1 year (8.4 ± 3.4 mmHg), and reintervention rate was high at 1 year (9.1%). However, there was a significant improvement in quality of life which was sustained at 1 year (mean KCCQ score improvement at 30-day was 24.1 points and at 1 year was 25.9).
