340B as it exists now is crucial to oncology care. In @JCOOP_ASCO, @ASCO releases updated policy recommendations for reform of the program:

I voted for Dr. Makary to head the FDA based upon his commitment to say ‘no’ if President Trump asked him to do something that would harm America.
Dr. Makary kept his word. He resisted Trump’s plan to expose millions of kids to the dangers of vaping.
I hope Dr. Makary will inspire others within the Admin to grow a spine & put the families of America ahead of blind loyalty to this President.

Breaking News: Dr. Marty Makary resigned as commissioner of the FDA, according to an administration official, after weeks of pressure and rumors that President Trump was planning to fire him. nyti.ms/3Pjpfjp

@Papa_Heme Most likely they will not consider to take the “toxic chemicals” we supposedly to prescribe.

You destroyed spirit airlines
Elizabeth Warren@SenWarren
I've warned for months that a @JetBlue-@SpiritAirlines merger would have led to fewer flights and higher fares. @JusticeATR and @USDOT were right to stand up for consumers and fight against runaway airline consolidation. This is a Biden win for flyers! apnews.com/article/jetblu…

As one of his longest and most vocal critics, I would frankly prefer to crawl into one of Comey's conversant shells than write this column. However, here we are. This indictment is unconstitutional and will not likely survive constitutional challenge...jonathanturley.org/2026/04/29/she…

@VincentRK Ai is excellent and very expansive in knowledge, but still missed about treating smoldering myeloma, without human, they will recommend cares for your patients without restraints.

I asked a question that I knew the answer for as an example.
This is not unique to Open Evidence. I see this limitation with all AI programs.
Medicine is complicated and when you want patients and doctors to use it, we need to know the limitations.
So much AI is being promoted (not necessarily by the company behind) as the future of medicine, better than doctors at MCQ tests and diagnostic puzzles, and more reliable. If I cannot get a right answer as an expert, then how can we be sure patients can decide which output is correct and which is not. Realistically how are patients supposed to know which AI chatbot to use for which question and what prompt works and what doesn’t.
AI is not intelligent. It is tripped up by questions that an average subject matter expert will know. AI at present retrieves and presents what experts have written. The answers sound great. Authoritative. Fast. And mostly right. But interspersed with errors.
AI is so good that its output in cardiology will impress a doctor who’s not a cardiologist. But a cardiologist will not be fully impressed. And digging deeper, AI answers to a question on say EP will impress all the doctors including most cardiologists. But not the EP expert who can spot the times when the AI answer is confidently wrong.

@HenrychihangFu1 Is there any limit how low can we push the dose of ven? I have a lady I have to back down ven to 50 on posaconazole for only 5 days and spaced treatments for every 6 weeks. 14 months now.
English

💣 AZA + VEN doesn’t fail—execution does.
Day 21 marrow decides everything.
Shorten VEN, not AZA.
Don’t wait for perfect counts.
Here’s how we induce AML in 2026 👇
Please let us know your approach.

@gtconway3d @SundaeDivine My daughter always tells people she went to a small college in Boston:)
English

@SundaeDivine I feel the same way. I never tell people I went to Harvard unless someone mentions Harvard first in which case I mention that I went to Harvard and go on to discuss how I never mention it unless someone brings up the subject.

California families are getting crushed by taxes.
As Governor, my first move: make the first $100,000 of income completely tax-free.
For everyone earning above that, a simple flat 7.5% rate.
No more punishing hard work—your paycheck stays in your pocket.
Let’s make California affordable again! ☀️👊#Califordable #GoldenAgain

@CattardSlim Is there any meaningful conversation at home in English? If you don’t use it, you lose it.

@AnilMakam @adamcifu Take it if you want, may get a calcium score, if it is good, number may not matter:)
English

@adamcifu Can mostly sum up the new LDL guidelines as:
Statins for everyone unless they dont wanna take it. For the rare undecided person, do more testing (IMO this is like the swing voter days before the election--exist but not many of em)
English

Let’s just say, there’s a lot not to like in the new lipid guidelines.

If someone is in hospital dying of cancer, and they want a coke… or an ice cream … or whatever
… you give it to them.
Doctor’s orders.

Calley Means: “I was at Stanford Hospital with my mom… they served her Coca-Cola.” “She was a diabetic dying of a metabolic condition from cancer.”

Absurd and inhuman violence is spreading ferociously through the sacred places of the Christian East, profaned by the blasphemy of war and the brutality of business, with no regard for people’s lives, which are considered at most collateral damage of self-interest. But no gain can be worth the life of the weakest, children, or families. No cause can justify the shedding of innocent blood.
