Hey @wesstreeting here's a simple way to save the NHS £924million annually: replicate Norwood GP Surgery nationally. In fact, make a low carb solid food diet an option in the NHS Path To Remission program (it currently isn't). The cost to roll this out is minimal, the savings are huge and the benefits to patients are immeasurable. Why can't this happen?

@Brady_H @AKoutnik Professor Tim Noakes has a study coming out i think this month in Endocrine Reviews that says you only need 20g/hour to fend of fatigue and that blood glucose level is the determining factor not glycogen stores as commonly thought. You might be interested in it.

120 grams of carbs per hour maximizes carb oxidation in elite marathon runners. Eight elite male marathon runners (national/international level) with personal bests averaging under 2:23 each completed a 2-hour run simulating marathon intensity under three carbohydrate intake rates: 60 g/h, 90 g/h, and 120 g/h. Ingestion of 120 g/h resulted in significantly greater whole-body and exogenous carb oxidation rates compared to 90 g/h and 60 g/h. Peak exogenous oxidation rates with 120 g/h reached 1.77 grams per minute (among the highest recorded in runners). But...higher doses increased nausea, fullness, and cramps. All athletes experienced moderate-to-severe gastrointestinal symptoms on every protocol, with 100% reporting significant stomach fullness regardless of dose.

Combining metformin with high-intensity exercise interferes with improvements in VO₂ max and vascular insulin sensitivity. Adults performing low- or high-intensity aerobic exercise for 16 weeks while taking metformin (2,000 mg/day) showed no improvement in VO₂ max. Those on placebo improved by about 6% on average. Metformin also blunted increases in macrovascular and microvascular insulin sensitivity seen with both exercise intensities, attenuated improvements in fasting blood glucose, and prevented gains in whole-body insulin sensitivity, even after high-intensity exercise. This is not isolated; other trials show metformin interferes with multiple exercise adaptations, likely via inhibition of mitochondrial complex I, which disrupts cellular energy sensing. Metformin remains an excellent, safe first-line therapy for type 2 diabetes. But caution is warranted when using it as a general “longevity drug” in healthy, active people, especially if it interferes with the single most powerful longevity intervention we have, exercise.

A new meta-analysis published in JAMA Psychiatry examined 50 different keto studies, totaling over 40,000 participants, that included depression and anxiety outcomes. The results? Modest improvement in depression symptoms across different populations and diagnoses in both controlled and real-world scenarios Greater symptom improvement was observed in trials that included ketone monitoring. A larger effect was seen in “very low carbohydrate” diets compared to just “low carbohydrate” diets. Improvements in anxiety were also observed in most studies, but with less consistency than depressive symptoms. In this video, @bretschermd breaks down findings from this study and what the results mean for psychiatry.
