@JoshCMandel These non-commercial tools put a lot of responsibility on the patient to do the prompt engineering around SMART on FHIR exports. This risks aggravating disparities for populations already struggling with access to primary care x.com/zakkohane/stat… This needs community input.

If you have to wait 4 months for a primary care visit or 2 months for mental healthcare, what are we comparing AI performance to? @NEJM #t1" target="_blank" rel="nofollow noopener">nejm.org/doi/full/10.10…

Love this! In a similar vein, I put together ctrl-c.fhir.me last month for a sort of "decomposed demo" at the Madison Literary Society. This tool just: 1. Connects to your SMART on FHIR endpoint 2. Downloads all the clinical notes to your browser (optionally filtered by keyword) 3. Copies all matching notes to your clipboard Then you can paste into your LLM chat interface of choice for explanations, summarization, sense making, etc.
seanno@seanno

1. Patient access to clinical documents and reports is fantastic, but they're pretty complex. 2. Generative AI models are, with caveats, incredibly good at translating this data for humans. Enter "explain my notes" About: shutdownhook.com/2024/11/10/exp… Try it: shutdownapps.duckdns.org:7078

On the same day @nytimes op-ed predicts @JetBlue getting worse, my incoming flight sat on the ground, full, for an hour and 20 minutes before getting to the gate at PHX. Delays are one thing but this seems positively #cruel.

I am very excited to announce I have been successful in installing and operating a full ChatGPT knowledge set and interface fully trained on my local computer and it needs no Internet once installed. There are no editors and there is no censorship. I am using Alpaca (crfm.stanford.edu/2023/03/13/alp…) from Stanford and Dalai Lama. The training model cost about $530 to build locally yet has the abilities of GPT 3.5. The software is free and open source and I am working on preconfigured packages for anyone to have local training and access to a LLM GPT AI. This model is now in a live connect with all of my other AI systems and the results have been absolutely stunning. I will be writing more about this soon. But today know, you will own your own AI and it will only answer to you.

@bewestisdoing @TrusteeHIE @PatientPrivacy @FlemingProtocol Longitudinal patient-centered health records are essential for clinical, research, and public health. Patients get to set priority across the three use-cases and exercise control via trustees and communities they choose. Premature optimization for one use-case is unnecessary.

@sendgoodcheers @agropper RE anti-trust scrutiny, Adrian's proposing that data-holders, data-brokers and data-providers shouldn't be able to construct monopolies precisely because they wind up abusing data rights of others. Lack of interoperability is one of the symptoms; the outcomes.

After self-hosting my email for twenty-three years I have thrown in the towel 😩 Email is now an oligopoly, a service gatekept by a few big companies which does not follow the principles of net neutrality.

@Sheldrake @csageland @hiproj Forced association with and surveillance by powerful global platforms for a large part of what we do every day is a human rights violation. @csageland @agropper @hiproj So questions relating to mediation forms, including the sources of meaning (values) and motivations informing the interfacial affordances? A fascinating field of enquiry!
