Now we do lithotripsy and stone retrieval.
Minimal pancreatitis in 8% patients.

Another @nhsuhcw patient saved from having a life-long cholecystostomy drain, as unfit for surgical cholecystectomy.
I've now helped 40 patients so they can live drain free by performing GUPTAS procedure, which involves putting a stent between the gall bladder & the duodenum.

Perc chole with subsequent interventions such as lithotripsy and lithectomy (including of the CBD stone) only if the patient is not a surgical candidate.

For these type patients we’ve moved to doing a combo of lithectomy and drain internalization (w 2 kissing stents if possible to prevent clogging), all perc via drain track. @AhsunR is the guru

A patient presents 3 months after laparoscopic cholecystectomy in which there were numerous spilled stones.
CT shows a phlegmon in a very superior and posterior subphrenic location (see images). The patient is moderately septic.

I have done 5 now with success in 4. 1 had chronic scarring around the stones which did not allow visualization.
