I’ve done it once and seemed to work. @aswinchari , your first principles only apply if you’re looking to decompress cord/roots. This patient has symptomatic pharyngeal compression, so why not decompress?

@drtintin99 @e1v1m1 @realbrainbook There is some kind of segmentation abnormality. Not sure if C1 anterior arch assimilated in to skull base or dens. Need to see lateral masses too. 💯% OC fixation, may also need dens resection. Seen descriptions of traction first, but never seen it first-hand.

I agree this is basilar invagination and probably platysbasia but scan isn't showing all of skull base to confirm. Questions from me: 1) Why invagination rather than impression? 2) Why C1/2 and not OC fusion given the alar ligaments are gone? @e1v1m1 @realbrainbook
