@armyemdoc @gruntdoc @TheSGEM Lay use of naloxone is not an argument for how a physician (or trained paramedic) should use naloxone. You’re a physician, man. Do no harm.

When it comes to naloxone opioid reversal, there are 2 schools of thought - give them just enough to increase their RR or totally reverse and wake them up. I'm in the latter. I rarely start with <4mg. There's a theoretical risk that their opioid could outlast the naloxone, they wake up, refuse transport or leave the ED, and then drop dead. That's another myth. In a study of 542 patients completely reversed by EMS followed by refusal of transport, there were 0 deaths within 48 hours. @AmericanAir Delayed for maintenance and plane switch by 3 hours. Then that suddenly Delayed last minute by 49 mins. Then we walk halfway down jetway and you pull us off for pilot time out. Then say customer service that way: closed. Go to another desk: closed. Directed to 3rd…

No one there. 2 hour wait for phone… I have two small dogs and the 4 elderly people I was helping.

The chemo nausea study was on 8-16mg per dose, so I start 8 for those REALLY bootin’ But in combo w others, esp in those with poor clearance issue, addition becomes multiplication of ADEs

I am sure most cases of pyelonephritis can be managed at home with oral antibiotics I think we admit too many cases #NHS

@benabo @spenserlang @johnbrucewalla1 @DrNeilStone It’s mostly if they’re on other meds or have predisposing arrhythmogenic comorbidities. Had some BMT patients with levaquin for ppx then got aspergillosis needing an azole. Heme/onc didn’t bother to look prior to DC, left w 3 QTc drugs… and VT in 3 days..

@johnbrucewalla1 @DrNeilStone <50yo w ED dose of CRO/CTX w 2L LR after POCUS to r/o hydro/Urinary obstruction? I almost always then DC w some Phenergan/Zofran to take 30min pre PO abx (EKG to ensure no QTc issues), w close PCP f/u of course! @benabo would agree I’m sure ;)

Sometimes I get frustrated with patients coming to the ER with a obviously benign rash, but then I remember that there are online physician groups thousands of members strong and 90% of the posts are pics of benign rashes asking for help.
