An alternative case scenario available to you and almost everyone—I'll paint the picture of the same one I've walked people through: Employee Bob was fortunate to work for a company fed up with paying huge premiums to a BUCA for fully insured healthcare coverage. It had converted to a self-funded healthcare benefit program (essentially pay-as-you-go with stop-loss insurance to cap costs no higher than the prior BUCA program). The company also had the foresight to directly contract with a Direct Primary Care (DPC) physician group, providing direct access to physicians/same-day or next-day appointments/on-site work clinic hours/discounted labs/discounted imaging/discounted medications/no-cost in-office procedures. This meant every employee enjoyed a long-term relationship with their DPC physician—who knew them well, served as a trusted resource for all medical matters, and was available by phone/text/email anytime. So... right lower quadrant abdominal pain strikes. The patient calls their physician right away on their cell phone (no overnight/weekend/Holiday delay in diagnosis/treatment), getting an order for labs ($10), a same-day office appointment for comprehensive history and physical exam (no charge), bedside screening ultrasound (no charge), confirmatory CT of the abdomen ($250), IV fluids if needed ($8), and IV/oral antibiotics ($20). By skipping the ER waiting room—saving 8–12 hours and a $2,400+ ER bill (more than two years of DPC membership cost)—the patient gets earlier care, when less intense intervention has a better chance of resolving it outpatient, allowing rest and quality nutrition/support for everyone. Same story every single week in Direct Primary Care….the choice for something different is at your fingertips. This is unfortunately true. Patients get sent to the ER because a CT or MRI would take a few days or weeks. But it need not be this way. Getting a CT or MRI or other workup in an ER is expensive. A doctor who owns his own scanner is cheap. Imagine if your spine surgeon owns his own scanner. You have an issue? Just call the office and they’ll probably open up after hours to get you in and take care of your problem the next day. That’s one ER visit saved. One less bed being taken up for a non-emergency. That’s what ownership does.

Can somebody explain to me why when im working a shift in the ER, even in my doc box, im not allowed to have any food or drinks in there (per JCAHO)? Like I can’t leave the ER to get food/drinks so often the only option is to bring something, but I have to leave the ER to consume it?! And families can bring McDonald’s and eat it in the rooms? What am I missing here??
