@CorKinetic I’ve not read the full paper but would be interesting to know if the testing highlighted asymmetries & if these correlate with symptoms. Also if court positions played a role here

I always like studies like this that challenge biases! Can we sometimes confuse capacity for producing force as capacity for tolerating it? pubmed.ncbi.nlm.nih.gov/23060653/

@O2 your online service is diabolical. I can only speak to bots and consistently get shut down mid conversation. Please respond with a way I can discuss my concerns with a human

@brian_drago_ Would you say they’re more of a training shoe rather than race given what you say about track sessions?

Nike Vaporfly 3 Review 🔥 Keeping it brutally honest (as always) with you. The upper on the shoe is straight WEIRD. But, to its credit, you don’t think twice about it while running. Snug fit, runs normal sizing. First impressions are: + it fits well on the foot, very little slippage. + best foot-to-ground contact in a Nike carbon shoe. + light, and feels that way while running. - firmer than previous models. It’s certainly fast, but it doesn’t feel like you’re cheating quite as much as the alphafly. Locking into threshold was a dream with this shoe. At 5:40/mi - 5:50/mi, it comes alive and responds when you want the speed. It’s the least clunky Nike carbon shoe - a good in-between compared to the ASICS metaspeed sky (firm) and the Nike alphafly (ultra cushioned). If I could pick an interval track workout shoe for every workout, this would be it. Hands down. Fast feels natural, but you can hold solid running form without over-relying on the shoe to do the work. Questions? Ask below 👇

@bellkneesurgeon Framing the conversation right is so important. Weight driven conversations typically cause a patient to shut off. Changing the narrative like this, can really help!

OA knee. I now “ address” the patient’s metabolism, rather than going on about weight. Improved metabolism equals better weight but also reduction in pain. 👇

@hjluks Great post, Howard. Discuss this most days but today was one of the first times I got instant buy in from a patient. Was a great conversation. Only if it was that easy all the time. 🤦🏼‍♂️

Address the system... not the symptom. Tendon and joint pain in adults is often a downstream consequence of a more significant systemic issue... usually something to do with our metabolic health. Joint pain is often blamed on mechanical wear and tear—like a meniscus tear or arthritis—but it’s not always an isolated issue. Many cases of adult joint pain are a manifestation of systemic metabolic dysfunction. A growing body of evidence, like the study linked below, highlights the role of metabolic health in joint pain. Conditions such as insulin resistance, high triglycerides, and chronic low-grade inflammation disrupt joint homeostasis, accelerating osteoarthritis and other degenerative changes. Elevated inflammatory mediators and poor energy substrate management don’t just affect your heart or blood sugar—they wreak havoc on your joints, tendons, and cartilage. The implications are significant: Treating the root cause—metabolic dysfunction—may be more effective than focusing solely on the joint itself. Simple lifestyle changes, such as improving diet, walking daily, resistance training, and maintaining a healthy weight, could ease pain, slow progression, and improve overall quality of life. Joint pain is often a symptom of something bigger. Address the system, not just the symptom. sciencedirect.com/science/articl…

@mboyle1959 I’m not surprised. Useful work but patient selection, like anything, is key

@mboyle1959 Agree mate. Like most things it’s patient/client selection. When used with the right person it can be great. If not, it can be counterproductive or even, dare I say it, useless!!
