IOC has an important role…but as I’ve said many times, I hate the idea of making a hole in something just to figure out what it is. If there’s already a hole in it, that’s a different story. @StJoesHPB this is how we do it and teach it:

Good points Though really not sure that IOC is the best thing to do when CVS cannot be achieved. Doesn’t make sense to poke a hole in something without having identified it. Also many issues with incomplete cholangio and misinterpretation. I would caution presenting it as a solution to difficult anatomy. @cutitoutPODCAS1?

I think the FDA approval benchmarks for all modalities including systemic therapies, devices, etc should be the same. That this therapy was approved based on practically very little evidence (if held to the same standards as systemic therapy)- makes its use in practice very problematic. Hope that’ll change in the future.
