MRI Diagnosis of Impingement of the Anterior Cruciate Ligament Graft, Intercondylar Roof Type: a chicken and egg situation A 21-year-old soccer player with a history of anterior cruciate ligament surgery 3 years prior, associated with a modified Lemaire anterolateral reinforcement, underwent an MRI due to anterior pain of 3 weeks' duration that did not prevent him from continuing to compete. The MRI revealed grade II chondromalacia patellae associated with iliotibial band friction syndrome, explaining his symptoms and the biomechanical origin of his condition. In addition to these alterations, there was increased signal intensity and decreased thickness of the graft in its middle third where it contacts the intercondylar roof, associated with loss of tension in the graft adjacent to its entry into the tibial tunnel, which is located partially anterior to Blomensaat's line, consistent with the radiological diagnosis of impingement of the anterior cruciate ligament graft, intercondylar roof type. The etiology of this impingement is a very anterior position of the tibial tunnel, as initially shown in the video. However, in this patient, a three-month post-surgical MRI demonstrates the correct tunnel location and the graft's normal morphology. This would indicate that the patient suffered at some point an asymptomatic partial tear of the graft, which caused graft insufficiency and anterior tibial displacement, erroneously interpreted on the actual MRI as a very anterior tibial tunnel. Does this mean that many of these radiological diagnoses are incorrect and that it was not a problem with the surgery but rather a sequela of an asymptomatic tear?

Update - L radicular leg pain & foot drop almost resolved Unfortunately symptoms evolved to become bilateral gluteal, pelvic & groin - 2 weeks ago When the pain got to 9-10/10 & I couldn't sleep / walk, I decided to forego watching the Hungarian Grand Prix & head to Urgent Care at PGH instead - raised CRP & neutrophilia MRI on Sunday at Fortius (amazing service) - raging bacterial disciitis & epidural abscess Don't tell anyone but it's affecting the psoas so I can't SLR 😆 So now in hospital awaiting PICC line and 2-3 months antibiotics 😕❎

Update for those interested! 16 days post discectomy Minimal pain - low grade 2/10 neural ache buttock and fibula head / shin Worse at night and morning but variable Numbness is more annoying! Tib ant power recovering Doing simple upper body weights typically seated or cable Iso rehab for quad, hamstrings and tib ant Stationary bike & elliptical trainer - easy steady state I’m behaving and listening to my brilliant physio Chris Whittle @puresportsmed
