I stepped out of a new patient’s consult to have this peer to peer call at the only time offered to me. You may remember an insurance company recently insisting that they’d never interrupt patient care for insurance matters. Reality Check…it happens all the time. The doctor I spoke with was a general surgeon who had never performed breast reconstruction. He declined to give me his name, citing concerns for his personal safety. He was polite, but ultimately told me that United considers nerve sparing mastectomies “experimental”. He would not provide a written rationale or references for the studies used to make this determination. But he kindly invited me to appeal. Sure. What a great idea. The patient and I have all the time in the world. Let’s do this all over again. Until we are seen and heard.

But what if we provide 'too much' supplemental oxygen? This can cause the loss of hypoxic vasoconstriction increasing perfusion to the poorly ventilated alveoli. (Maybe a better term would be HYPERoxic vasodilation! h/t @sargsyanz for suggesting this perfect term!) 13/

This is irresponsible and going to cause serious harm to children. This is what can happen to a baby’s brain without vitamin k at birth. He has no qualifications to make these recommendations and has never had to sit with families and deliver awful news.

Unpopular opinion but if a trainee asks you a simple medical question that you can answer in 2 min during rounds plz don’t ask them to look it up themselves when you have the needed information at your immediate disposal. It’s both discouraging and inefficient. Agree? Disagree?

@tchen091 Sure! I used the phrase “might have UTI” because the presence of pyuria does not confirm the diagnosis. Basically, if there is pyuria plus symptoms, it’s suggestive of UTI. No pyuria means UTI is extremely unlikely. Pyuria with no symptoms is also probably not UTI.
