It has been an amazing transplant year at Mayo Clinic in Arizona. We surpassed the volume of previous years. Our abdominal transplant programs completed milestones in liver (322), kidney (506), kidney/pancreas (52), and pancreas alone (2). New innovations abound - robotic kidney transplantation and living donation, machine perfusion of organs, advances in organ retrieval, transplant oncology, training and educational excellence, and research initiatives that will shape the future of transplantation. Our programs have continued to provide our patients with excellence in clinical outcomes, hope, and unparalleled experience. Lazarus Syndrome: History and Clinical Aspects Lazarus syndrome, also known as autoresuscitation, is a rare medical phenomenon where the heart restarts spontaneously after failed cardiopulmonary resuscitation (CPR) and a declaration of death. As depicted in the image, it often occurs due to lingering drug effects, though full recovery is uncommon. Lazarus syndrome was coined in the 1980s, named after the Biblical figure Lazarus, whom Jesus raised from the dead after four days. It highlights over 65 reported cases, emphasizing its rarity and defiance of death. This aligns with early documentation, where the term evokes the resurrection story from the New Testament's Gospel of John. It is defined as the impromptu resumption of cardiac rhythm in a patient declared dead following cardiac arrest, often with non-shockable rhythms like asystole or pulseless electrical activity (PEA). The exact mechanisms remain unclear, but proposed causes include: PRESSURE DYNAMICS: Buildup of intrathoracic pressure from vigorous CPR or hyperventilation (auto-PEEP), which, upon relaxation, allows venous return and heart refilling, triggering electrical impulses. Pharmacological delays: Impaired drug delivery (e.g., adrenaline) due to poor circulation during CPR; effects manifest post-cessation.aa2da5 METABOLIC FACTORS: Hyperkalemia, hypovolemia, hypothermia, or intoxications, which can reversibly suppress cardiac function. OTHER: Unobserved minimal vital signs or myocardial reperfusion from dislodged plaques. Symptoms manifest as sudden return of pulse, breathing, or movement—e.g., a patient waking in a morgue. Diagnosis relies on post-event review, with no specific tests; it's retrospective. Treatment focuses on prevention: gentle ventilation to avoid hyperinflation and 10-minute passive monitoring after stopping CPR, as recommended by European Resuscitation Council (ERC) and American Heart Association (AHA) guidelines. Outcomes vary: Survival ranges from minutes to months, with full neurological recovery in about 8% of cases (e.g., six patients achieved complete recovery without deficits).Around 45% experience good recovery, but many face neurological issues or die soon after. Ethical implications include risks to organ donation and legal liabilities, as in cases where "dead" patients revive during autopsy preparations, prompting lawsuits. Underreporting due to fear of professional repercussions likely underestimates prevalence.
