CLINICAL ASPECTS

It is defined as the impromptu resumption of cardiac rhythm in a patient declared dead following cardiac arrest, often with non-shockable rhythms like asystole or pulseless electrical activity (PEA).
The exact mechanisms remain unclear, but proposed causes include:
PRESSURE DYNAMICS: Buildup of intrathoracic pressure from vigorous CPR or hyperventilation (auto-PEEP), which, upon relaxation, allows venous return and heart refilling, triggering electrical impulses.
Pharmacological delays: Impaired drug delivery (e.g., adrenaline) due to poor circulation during CPR; effects manifest post-cessation.aa2da5
METABOLIC FACTORS: Hyperkalemia, hypovolemia, hypothermia, or intoxications, which can reversibly suppress cardiac function.

Ethical implications include risks to organ donation and legal liabilities, as in cases where "dead" patients revive during autopsy preparations, prompting lawsuits. Underreporting due to fear of professional repercussions likely underestimates prevalence. Overall, it underscores need for cautious death certification in clinical settings.
