This is presented by NYC as an advancement in legionnaires response, but it's not. Publicly naming every building with even 1 cfu/ml, **VIA PCR**, is theater; 84% of cooling towers will have "detectable" legionella via PCR, per a study by CDC on this. And, *as they say right in this quote*, the best and fastest approach is simply to test and boost the disinfectant ("...already an effective measure"). A bigger problem is that there is no plan for on-going surveillance and requirements regarding disinfection and testing. Legionella come right back, so a temporary "cleaning" doesn't do anything if a regular inspection and review of disinfection isn't part of the plan. So when they say here that "the full cleaning and disinfection reduce the risk that any living legionella bacteria...will grow back," they are incorrect.

Two coworkers each eat a slice of pizza at eleven at night. One had an ordinary Tuesday. The other spent it grinding through back-to-back deadlines, jaw clenched since breakfast. Same clock. Their guts are not the same. The study behind that claim never tested everyone who eats late. It looked at people who took in more than a quarter of their daily calories after 9 p.m. while also carrying high stress. That group was one and a half to two and a half times more likely to develop constipation or diarrhea, with a measurably less diverse gut microbiome, the kind researchers call dysbiosis. The lead researcher's own read on her data: late eating by itself did not drive that pattern. Stress did the real damage, and eating late just made it worse. The finding was presented at a medical conference this spring and has not yet been published in a peer-reviewed journal. It says something real about stress and timing together, not a curfew for your fork.

When emerging evidence challenges an #FND diagnosis, the patient should come first, not the diagnosis. Better testing, thorough differential diagnosis, and a willingness to reconsider help patients. Defending an FND label at all costs does not. Put people before diagnoses.
