I have only seen one endoscopy done where I currently practice. When I saw the images and the prescription, I thought, “Interesting.” Since then, for patients with persistent or concerning symptoms suggestive of peptic ulcer disease or significant upper GI pathology, I recommend endoscopy after the initial primary management and assessment. Interestingly, I have recommended it to several patients, but many never go ahead with it. What usually follows instead is repeated use of drugs like Omeprazole brands such as Meprasil, or medications like Esomeprazole, Gaviscon, Acipep, and similar agents. Many of such prescriptions come from physicians themselves. Endoscopy is the gold standard. Nobody is disputing that. But the real-world question is this: in Nigeria, how many physicians can confidently say: they routinely perform or directly arrange endoscopy, have easy access to the facility, strongly recommend it, and the patient actually accepts and pays for it?

Let me tell you something 😭😂 If my wife leaves the kids with me and I mistakenly lose guard and the children end up doing this… It’s either: - we clean those chairs completely, - contribute money and buy another one, - or all of us relocate to Jalingo before she comes back 😩💀 Because if she enters the house and sees this scene… both me and the kids already know say peace treaty no dey again 😂😂
