Mandatory to always look at the pressure curve before injecting contrast. It’s so basic and a preventable complication.

Interesting case that reminds us to remain aware, but the most likely explanation for hypotension in intermediate-high risk PE is right heart failure and the most likely explanation for sub-optimal anticoagulation in either direction is IV heparin, which we know now is so frequently out of desired range. Many things to learn from every PE case, the great humbler.
