Should we be rethinking tremor that does not respond to levodopa? Some folks quote 20-40% for this phenomenon? Swinnen and colleagues turn this notion on its head in their recent @MDCP_Journal paper were they argue with data it is actually ~4%.
Key Points:
- The authors comment that 'tremor in Parkinson's is commonly regarded as less responsive to levodopa.'
- 526 folks.
- All were screened for possible deep brain stimulation placement.
- Improvement in tremor 86.8%.
- Improvement in bradykinesia 45.7%.
- Rigidity in 67.0%.
- Full resolution most frequent for tremor 67.9%.
- Levodopa-unresponsive tremor (improvement <25%) only in 4.0%.
My take: Is it possible that we have been over-estimating the number of folks with levodopa unresponsive tremor? This paper would say YES. The tricky part, as the authors point out, is that tremor may also be a component of motor fluctuations. There are also cases where the tremor is partially responsive and if you catch it at the 'right moment' it appears to be completely responsive. This paper by Di Bie's group was very nice as it suggests that the proportion of patients w/ L-dopa-resistant tremor of 4.0% may be substantially lower than previously reported. So next time you see a case of possible medication refractory tremor think tabula rasa – a clean slate. Do not be biased by dogma, follow the data.

A person with Parkinson's disease can't walk because of severe freezing but put him on a bike and he is fine. Great case by Snijders and @basbloem Love the explanation: 'This striking kinesia paradoxica may be explained by the bicycle's rotating pedals, which may act as an external pacing cue. Alternatively, the motor-control mechanisms involved in gait as compared with other activities engaging the legs, such as cycling, could be affected differentially in Parkinson's disease.
