Are familiar with Sarno / mind-body medicine or psychosomatic pain by chance? Considering your archetype (founder, high intensity, etc) and anxiety background, there's def connections. Biggest headlines are back pain but can manifest is so many ways. Have seen quite a few recovery stories from folks with long COVID, POTS etc.

OP's health issues are more or less what I've been dealing with the last 18mo. Here's what I've tried that's actually worked so far: - If you have ANY positive sleep apnea result from sleep study, do WHATEVER you can to get a CPAP. Apnea = suffocating while you sleep = body dumps adrenaline XX times per hour while unconscious => anxiety, panic, atrial fibrillation, and more. - Control + track diet, high protein, 30g+ fiber, moderate calories, try to slowly cut down on body fat to normal levels. Don't crash cut. - Try the UTAH ADAPT cardio and strength protocol. It helps to slowly recondition cardiovascular + nervous system so POTS-like symptoms drop away. - Do EXTENSIVE daily myofascial release exercises across head, neck, shoulders, etc. Will take you a LONG time to get these areas looser and will also improve sleep drastically. - Take Allegra (H1 blocker), Famotidine (H2 blocker, aka Pepcid AC), and Histaminx (Mast cell support). This is the triple threat for histamine overload. - Go to sleep at the same exact time every night, when you wake up immediately get into motion for 20-30min in sun, as much of body exposed as is possible. This is a really weird 'package' of symptoms that a LOT of people are experiencing, myself included. Hope it helps. I've made incredible progress over the last 3 months alone on this and much more to come.

ok let's try this for some years now, I have been struggling with persistent fatigue (that does not improve with sleep), joint pain & muscle tenderness (workouts also cause notably slow recovery), persistent neck/shoulder/upper back tightness & tenderness (neck pain improved slightly with Sarno), and of course broader inability to focus (been diagnosed with ADHD). I have learnt to work around it, but it still feels like I am operating at ~20% of what I can. Some bullets: - No single notable event/cause - in my early 30s - Sleep apnea test only suggested borderline sleep apnea - I have chronic allergic rhinitis (histamine intolerance?) which likely contributes to the apnea and causes long bouts of mouth breathing - Frequent bloating, either with or without meals - Sugar cravings - I am a bit overweight with persistent visceral fat, though I also have fairly high muscle mass - Sweat easily, some degree of heat intolerance - Have MTHFR genes - Vit D is persistently low (~15), B2 was low (2.5μg) though I assume some of it might be stored in fat tissue - Homocysteine was at 13 last year, now lowered to 11 - HRV stuck around 20ms - Paradoxically strong glutathione status overall - Fecal secretory IgA very high (>7500mcg/mL) - Very high presence of gut bacteria across the Bacteroidetes phylum (Bacteroides uniformis, Phocaeicola vulgatus, Barnesiella spp., Odoribacter spp.) - Energy metabolism comes out low across Citric Acid, cis-Aconitic Acid, Isocitric Acid, α-Ketoglutaric Acid, and Succinic Acid - Older glycans test showed high levels of inflammation Things I have tried: - AIP diet for four months (slight improvement in focus, RHR went from 69->63, but otherwise no big felt improvements) - MTHFR supplementation, with no felt improvements - Supplementing 10g creatine daily (no felt improvement) - Tested negative for SIBO and Candida - Thyroid looks good? (latest labs with FT4 at 1.43ng/dL and TSH at 1.0400 μIU/mL) - For ADHD tried ritalin, and it notably only worked the first time, then stopped (I cannot try heavier stims due to having the Long QT gene) - Meditation (helps) - Antihistamines in case of histamine intolerance (no felt improvement) What I can’t figure out is: Is this primarily: A) Sleep-breathing/autonomic-driven B) Gut-driven inflammation C) Methylation + B vitamin related E) Some combination of all of the above If you’re someone who’s dealt with a similar cluster (fatigue + ADHD dysregulation + gut issues + high-ish homocysteine + low D + borderline sleep apnea), I’d genuinely appreciate any ideas or insight into what to try next.

To everyone asking for details: I helped Neal and Ian by regulating their immune systems. That is the only information I can give at this time as what I am doing is not available elsewhere. If it were as simple as a supplement you could order off amazon, I would tell you. Once I have run formal studies and confirmed efficacy and a lack of side effects, I will discuss further. Applied Determinism is completely separate - it is useful for living a more peaceful life, not for addressing chronic inflammation.

More random observations as I start my 4th month on GLP-1 (tirzepatide) PROS 1. Weight loss has plateaued, which I'm fine with. I have visible abs, been called "chiseled" more than once (tyvm). I don't need/want to be tot shredded. 2. I basically only need fruit, protein shake and a hearty dinner. 3. Chronic back pain 90% gone, thanks to lower inflammation 4. Really good bm's/digestion, like clockwork 5. Speaking of inflammation my preference for super spicy food doesn't come back to bite me anymore in the bathroom. Strange. 6. Small aches and pains are gone too CONS: 1. Occasional BAD heartburn. The slower digestion means it can catch up with you later on acidic meals. Punishes even a little overindulgence on rich food. 2. Slightly less motivation to really push cardio in the gym since I am really happy with my weight. A feeling of "I'm good" 3. I do notice a little more wrinkling in my face with it being thinner. Still worth it. 4. Still not sure about long term usage. We'll see. AMA

Daniel also has helped me massively improve debilitating post viral / Long COVID symptoms over the last month. I believe he has the workings of an entirely new framework through which we can view immune and autonomic nervous system health that can massively improve these difficult to treat and brutal conditions.

I'd like to thank @daniel_rossett for his help in my recovery from the POTS version of Long COVID. Daniel was key in bringing me back from highly disabled and suffering to being able to do what I want to again. This X account is mostly focused on ML / AI. From that point of view, many of you know that in December 2024, I wasn't able to do the test of time award talk at NeurIPS, even by video call. Daniel started working with me in March 2025. By April, I started to have days of no POTS symptoms, by June I was off all heart rate lowering medications, by September I was back to work. I'm back to full exercise, running, lifting weights, mountain biking, and have even done things I hadn't done before I got sick, like riding Whistler Mountain Bike Park. I'm now getting the word out to help Daniel build a company that will bring this approach to more people.

You’re spot on 🎯 This is why I’m a strong believer that grounded generalism will be ultra valuable in this new age. You have to be able to see across silos and situations to 1) structur modern systems properly and 2) command agents within those systems. This is essentially my whole pitch for Square’s APM program. much needed context. @blocks is on the forefront of this AI productivity push. Many of us don’t write code anymore but here’s the nuance missed: - Taste and expertise is more important than ever. AI automates the boring parts to let you focus on the bigger picture - Normal development pipelines, rituals, and expectations are breaking from AI first development - Platform and internal tooling teams are more important than ever and are understaffed for what’s coming - Initial architecture and AI rules is critical. If you are not building on good foundations your AI will write 5 different pillars of sand.

My only advice to CEOs this year.. Hire tinkerers. Make it high status. Find ones that will explore the edges and (ideally) naturally gifted at teaching people. Empower them to freely roam across the org and fix large problems that can be automated. All your execs will complain that these tinkerers don’t understand scale or systems (rollout being a favorite word). Listen, and ignore them. Or even better give them budget to hire a tinkerer to achieve their targets. Give these tinkerers ambition, purpose and hard targets and watch them fly.

Hey @GregoryFStewart! Inspired by your recent interview with @patrick_oshag, I wanted to share some thoughts on Ladder. I recently joined Team Cruise Control and am absolutely loving it so far! It's already transformed my habits, mentality and soon personal health. I'm really excited about what's to come. So much so that I have some ideas I wanted to share. Senior Fitness Older adults want AND need to be active. Truthfully, nothing is catered specifically to them in a way that's 1) impactful and 2) exciting. I'd love to see several older (50-65) coaches with plans and focuses that are specifically catered to our aging demographic. Advertise on FB. Partner with a gym network like the YMCA which already has a substantial senior crowd. There's even potential to tap into Medicare Advantage fitness funds (more complex). I think this would be a big hit and make a big impact on health outcomes. Swap Workout Visual As you know already, form and direction are really critical to a productive workout. There's been several times I've tried to switch my workout based on available equipment or ability, but decided not to because I didn't have a Coach Andre visual to follow along with. I know this is likely a media capture problem (he's mentioned needing to film at HQ) more than it is technical, but I wanted to highlight it anyways. I know others feel the same way. Nutrition Simplicity After some trial and error, I'm convinced the AI image calorie calculator is inaccurate. It's been wildly off several times. It might be great for super simple things (ie. ground beef and rice), but that's how Twitter influencers eat. Real plates and meals are substantially harder to analyze that way. Could it be more accurate by taking an ingredient picture with a servings estimation and using the finished product to verify? It's a hard problem, but I know it's more complex than just snapping a picture. Due to this, I've been mostly entering my ingredients manually and portioning them accordingly to get a rough serving calculation. Having to sort through the generic and branded ingredient lists can be frustrating. Too many options. I know some people are potentially concerned about the nutrient/mineral composition of different branded choices, but I'd venture to say that's the minority. This improvement might look like a "standardized" food directory that just has a few choices for certain foods (ie. different ground beef ratios)? I think the standardization of your food ingredient repository would be a huge win and worth the data capture challenge. Hydration Tracker Drink more! More! It's hard to stay on track with water consumption. I really wish Ladder made input (ie. shortcut, one tap button), tracking (ie. progress bar) and reminders easy. It would keep me motivated and data-driven. It would also nudge folks towards better liquid selection (see ya soda!). Maybe it can be gamified in a way that's fun? As you can tell, I'm excited about the potential of Ladder. What do you think?
