Proud to share this work. Complications are inevitable in endoscopy, but how we respond to them, support our teams, and learn from them is a choice. I hope this is a useful read.@BritSocGastro @ESGE_news @PhilSmithIsBack

Endoscopy complications don’t end when the procedure does. 🔎⚠️🧑‍⚕️ This @FrontGastro_BMJ article explores the implications of endoscopy complications and non-clinical management strategies — from team impact and communication to governance, learning and support. 📋💬 Essential reading for endoscopy units aiming to improve safety culture beyond technical outcomes. Timing isn’t just clinical. t’s human.” Timing should be part of complex procedures consent. 👉 Ask: “Is there anything important coming up we should plan around?” If complications lead to hospitalisation, the impact isn’t just physical. it’s psychological too. #GItwitter

In the UK, the nasobiliary drain (NBD) is less commonly used, but it can be a very effective tool. Sharing a video demonstrating setup for jejunal loop distension in EUS-GJ using a nasobiliary drain + foot pump. Simple setup. Controlled distension. #GItwitter

We recently performed an EUS CDD in a small duct using the push-pull technique with wire traction. This reinforces the value of teamwork & debriefing. Sharing a video debrief with my colleague Roderick discussing the steps, challenges, and key learning points. #GITwitter

How do you loosen the bumper the day after PEG tube placement? Some people just pull the bumper back, using the patient’s abdominal wall as traction. Ouch. Here’s another way - put the gauze UNDER the bumper after placement (!!!) - remove it when you see them next AM and instruct or an nurse to never replace. This effectively loosens the bumper without pain. Apologies if this is super obvious but it took me 4 years to figure this obvious move out.

At #SCIES2026, @drmwjames shared a message that every endoscopist should hear. Complications are not just statistics. Over a career, they become personal. The goal is not perfection. It is safer decisions, better systems, and continuous learning. #GItwitter

@drkeithsiau at Stoke complications symposium. Our job is not to hold information back. It is to make it understandable. When patients get the right information, they make better decisions & become true partners in care. This is how trust is built. #SCIES2026 #GItwitter

We see the benefit of having a surgeon who is a PB endoscopist in the team. The trainees also benefit significantly. @javlatif hope you are learning EUS as well. To train in ERCP/EUS & surgery will be difficult for many surgical trainees in the future

1/2 Dear Friends, I never wanted to become a doctor. I always wanted to write and tell stories. And yet, over the years, medicine gave me the most extraordinary stories I could never have imagined - stories that lived in hospital corridors, outpatient rooms and inside terrifying ICUs; in the trembling hands of a father carrying his jaundiced daughter, in the silence between a prognosis spoken and a family's world shattered. Today, I am proud and deeply moved to announce my first book, The Liver Doctor: Stories of Love, Loss and Regeneration, published by @HarperCollinsIN . This book is where my two worlds finally collide. My childhood love for writing and telling stories. And my adultdhood, as a medical doctor. Through the lives of real patients and their families - their courage, their grief, their impossible choices - I tell the story of the most misunderstood, most indispensable, and only self-regenerating organ in the human body: the liver. But this is not just a medical book. It is a journey through ancient myth and modern science, through Prometheus and Wilson's disease, through Mesopotamian clay tablets and liver transplant waiting lists, through the history of healing itself. I wrote it for doctors, so they may remember why they chose this life. I wrote it for patients and families, so they may know when to fight and when to find peace. I wrote it for myself, to make peace with what I have lost and what I will lose. This book shoulders that one truth I have learned in all my years at the clinical bedside: I did not become a doctor to help people cheat death, but to help them understand it. This book is my offering - to medicine, to storytelling, and to you. Lose yourself in these pages, as I have. Pre-orders are open now The Liver Doctor : Stories of Love, Loss and Regeneration - amzn.in/d/0duTenmW

GLP-1 receptor agonists are increasingly used to treat type 2 diabetes and obesity, and trials have shown reductions in cardiovascular risk and slowing of kidney failure. Adverse events are mostly gastrointestinal. Read the Review Article “GLP-1 Receptor Agonists” by Clifford J. Rosen, MD, and Julie R. Eight Couinaud liver segments are difficult to remember. Try my technique, let me know if it helps! Basically they are similar to the numbers on the clock 🕝 with "6" being the lowest segment and on the right My handdrawn figure to help understand (or to amuse) better
