The deck is stacked against physician ownership. Physician-owned hospitals are outright banned under Medicare. They’re excluded from 340B. They lack the tax breaks handed to big hospital systems. The playing field isn’t just uneven. It’s rigged. And yet, they thrive. These facilities deliver better outcomes, higher patient satisfaction, and happier surgeons. I had a blast talking to one of the pioneers of physician ownership, @eapotts, on the @NSGYPodcast.
Neurosurgery Podcast@NSGYPodcast

Ever notice how doctors don't *own* hospitals? We work in them, we manage them, we make them possible - but we can't *own* them, here in the USA. Ever wonder why? In this conversation with the tremendous @eapotts, we explore some of the story behind the ban on physician-owned hospital, current regulations, and the reality of life, business, and patient-care at a hospital owned and operated by the physicians. Spoiler: it's pretty great. Patients love it, doctors love it, staff love it, and the outcomes are stellar. Thank you to returning champion @DrDiGiorgio for insights on policy and politics surrounding physician-owned hospitals. podcasts.apple.com/us/podcast/phy… open.spotify.com/episode/6AU4ux…

This was just an awesome experience 1. Melbourne rocks 2. The best talk of the conference was delivered by a patient who, in 10 minutes, made us cry, laugh, and inspired 3. @GutFailure, @mariej_sinclair, @bc_diet et al are changing the world

The annual @Austin_Health ACTER Advanced Liver Disease Symposium held on the weekend was another outstanding success. 187 delegates were privileged to see @ebtapper drop some of his cirrhosis clinical pearls, along with a number of presentations from local experts.

Sometimes those busy months during delta feel like a different life. The memories. A different reality. A movie. I’m watching them but not in them. This seems kind of dissociative.

If your loved one is over 80, yes, I know, they WERE living independently before this new awful thing happened to them. This will no longer be the case. As we age we live closer and closer to our physiologic reserves until we are basically living at them. If you have no reserve, you can’t rally. You won’t be the same after. If you survive. Setting realistic expectations for you guys … If you want me to “do everything.” And I will, if you tell me that’s the direction we are going in, and I’m pretty good at saving people, remember it’s not on the rest of us to figure out the final details then where your loved one will end up living or how to get them everything they need in their new situation. Keep this in mind when you ask for what you ask for.

I tried to do the same Fenix celebration as Tadej Pogačar 🦅 But I want to also say a few words next to it. In the Tour de France with Vingegaard versus Pogačar, we have seen rivalry for ages. Again. And every time there is this question to me, who do I like? 1/5

Today my team sat down with our insurance broker. My health insurance premium for my employees went up 30% for our renewal. The government cut their pay to me and all doctors by 3.37% this year for the first two months then reduced the cut to 1.66% for the remainder of the year. I’m tired of paid mouthpieces telling Congress and Americans that doctors are the reason for our high healthcare costs. If you agree with me, share this post and let your friends and colleagues know where the money in healthcare is going. Hint: it isn’t to doctors

Results from #PROTECTION presented #CCR24 & published @NEJM. - DB RCT of amino acid infusion vs placebo in n=3511 people undergoing cardiac surgery w/ bypass. - Reduced incidence of AKI (26.9% vs 31.7% NNT=20) & need for RRT (1.4% vs 1.9% NNT=200) Potential game changer! 🧵 1/

This may be my favorite episode of @CritCareTime we’ve done yet! @Askins_Razor & I go DEEP discussing diuretics: how do they work, when to use them, drip vs bolus, & most of all why a negative fluid balance is so beneficial. Plus rants on CVP & MIVF Don’t miss it!

@CritCareTime @ross_prager Speaking of a procedure a lot *don't* currently perform, maybe discuss percutaneous ultrasound gastronomy sometime? Seems like a way to increase efficient patient flow in the ICU, especially for those who already perform bedside tracheostomy: youtube.com/watch?v=ekaw-6…
