Much of the resistance to coronary artery calcium (CAC) testing is not purely evidentiary. A positive CAC scan often reveals atherosclerosis without an indication for immediate intervention. That forces clinicians to explain risk, emphasize prevention, and tolerate uncertainty rather than reflexively performing procedures. For cardiologists trained in an intervention-driven system, and for patients who interpret “abnormal” as “fix immediately,” that can be uncomfortable. CAC frequently requires explanation and restraint rather than intervention. Let’s be clear about the issue - the issue is that it creates a communication issue and need to prevent unnecessary testing for spineless doctors who stand to gain from unnecessary tests and procedures. Because it makes “work” and because interventionalist are ill equipped it becomes easier to criticize the testing, when the data doesn’t support the emotional tenets of the debate. CAC is not a perfect test. A score of 0 does not mean “no plaque,” and a positive score does not guarantee an event. But the claim that CAC “fails in both directions” is not supported by the evidence. 1. CAC improves risk classification beyond traditional factors In the MESA analysis, adding CAC to standard risk factors significantly improved risk reclassification.
