PSA-only biochemical recurrence represents a highly heterogeneous disease state, but clinicians can generally distinguish between two main groups: patients with slow PSA kinetics, low PSA levels, and intermediate-grade disease, who are appropriate for surveillance or local salvage approaches, and patients with rapid PSA kinetics, high PSA levels, and high-grade disease, who require prompt, intensive systemic therapy such as ARPI plus ADT for at least nine months.
