@CMichaelGibson It’s a problem. Clear advantages from more readily accessible information. But unintended consequences of being locked into screens & not focused on human beings in front of us. Having a scribe has materially changed this for me. The amount of extra eye contact is transformative.

@elonmusk More Americans need safe affordable access to GLP1 agonists which will shift patients toward healthier food choices. At a critical mass, there would be spill over benefits to individuals not on GLP1s due to shifting consumer demands resulting in more healthy food availability.

Anyone still uses QCA? I still believe it is somewhat helpful in avoiding the occular overestimation of stenosis severity, here this 30% stenosis was described as “significant” by a colleague before the QCA

The business school version of how Doordash gained market share so rapidly is through clever strategy: targeting the suburbs where others weren't operating, acquiring untapped supply. That is not wrong, but it is also not all (or even most) of the real story. The actual story is that the team executed relentlessly. They made deliveries just a little faster and more reliable every week. They scrutinized the quality of every one of their restaurants and dashers. They optimized cost out of the system and gave it back to customers in the form of Dashpass (which launched in 2018 at the start of this chart). Over time, there was just better selection and faster, cheaper, more reliable delivery on Doordash, and consumers stuck. Virtually every startup is like this. There is a neat version of the strategy that fits into a 5-minute case study. But get under the hood and you'll realize that a lot of success is just showing up with grit and attention to detail every day.

#TAVR in extra large AV annulus, poor surgical candidate. Bicuspid with R/L fusion, Diameter 34.3 mm, area 925 mm2, perimeter 108 . LVOT 984mm2. #TAVR with 29 #Sapien 3+ 5cc. Excellent final result with no PVL. @RushMedical

@Paddy_Barrett Stage I hypertension, defined in the 2017 ACC/AHA guidelines as >130/80 effects even more and those relatively small differences spread across very large populations make a big difference. Stage I HTN w/ low cardiovascular risk is perfect for lifestyle change

I like the dumbbell shaped balloon & ability to sequentially add volume Basing balloon size on height seems like something we could improve upon w/ CT & crossing w/ agilis & wire would feel more controlled. Wish I could use balloon by itself

But have done a handful since. Seems like such odd technology in the age of TMVR, but here it is. Created this step by step cheat sheet for myself which may be helpful

All I see is a sheath and something inside. Don’t see balloon. I have seen Zero BMV during fellowships. I have not seen even one patient with rheumatic MS in last 12 years in USA. You don’t confirm with angio/pressure/Echo before advancing balloon in sheath? I would imagine balloon should never be in pulmonary vein.
