1/14 Why can't you use direct oral anticoagulants (DOACs) in patients with mechanical valves (MVs)? DOACs have been one of the most important advances in my career. And yet, the presence of a MV is one of the few contraindications. The reason highlights the unique nature of thrombus formation in those with a MV and provides insights into the evolution of human hemostasis.

Just out in AJSP – a paper from our group describing a MAJOR new oddity. Tumors that normally arise in the jaw from tooth🦷precursors (ameloblastoma) – but as lung primaries 🫁! This was a neat collaboration b/w MSKCC, Mayo and UTSW where these cases were encountered independently, and we put our heads together with the help of original contributors and fellows to tackle this puzzle. Time will tell if these are truly exceptional or have been underrecognized. journals.lww.com/ajsp/fulltext/… @JLHornick @ENTpathology @MSKPathology @PulmPathSoc @yro854 @thoracicpath @Path_Matt @SansanoValero @AndreMo77438628

Something interesting happens in the altcoin market when interest rates move toward the lower end of the cycle.

Let me make this very clear: Big Banks (think JPMorgan Chase, Bank of America, Wells Fargo, etc.) are lobbying overtime to block Americans from getting higher yields on their savings—while trying to block any rewards or perks from being given to customers. These banks, and others, pay rock-bottom rates on standard savings (often 0.01%–0.05% APY), even as the Fed pays them 4% or more. This massive spread fuels record profits, with almost none passed back to their customers / everyday depositors. Today, the banks are desperately targeting crypto/stablecoins, where platforms plan to offer 4–5%+ yields or rewards. The ABA and other lobbyists are spending millions trying to ban or restrict those yields via bills like the Clarity Act, crying “fairness” and using words like "stability"—when it's really about protecting their low-rate monopoly and preventing deposit flight. This is anti-retail, anti-consumer, and straight-up anti-American. Next time you see a big bank dropping billions on a shiny new Midtown Manhattan HQ, you know exactly where that money comes from: the non-existent interest rate they “pay” you! Fortunately, the big banks are losing this fight as customers wake up to the games… @worldlibertyfi

I had a feeling today would bring an unexpected diagnostic twist. The patient presents with a solitary lung mass and a remote history of “thigh surgery,” though the nature of that prior procedure remains unclear. As I reviewed the morphology, a familiar pattern began to emerge—does this architecture remind anyone else of a particular entity? @USFpathology #Moffitt #lung #path #pulmpath #thoracic #thoracicpath #PathTwitter #MedEd One of the most rewarding aspects of thoracic pathology is the remarkable breadth of disease processes it encompasses. Each case offers a new opportunity to integrate knowledge across organ systems—whether encountering an unusual metastatic pattern, a previously unrecognized systemic manifestation, or a subtle variation in tumor biology. The constant interplay between pulmonary pathology and the wider landscape of human disease ensures that the field remains both intellectually stimulating and clinically impactful. Thoracic pathology truly exemplifies the excitement of diagnostic medicine: every slide is a new question, every pattern a clue, and every day a reminder of how dynamic and interdisciplinary our work can be.

@ZachRector7 Zach! Glad you are doing well. I’m 40 and had open heart surgery to replace my mitral valve (mechanical valve) last year. Being a physician, it was very eye opening becoming the patient. I enjoy listening to your content. Praying for you.

@PathologyKate What about Warthin-Finkeldey giant cells? Classic for measles but also seen in HIV, Kimura disease, and other entities. Just a thought.

@PathologyKate This is great! Dr. Smith! My mentor. The man, the myth, the legend. Tell him I had a microcystic/reticular schwannoma recently. First one I’ve ever seen in real life. And tell him hello for me.

After 9 years of training, a pathologist in 2024 will get paid $35.36 to diagnose a person’s cancer, compared to $45 in 2000. That is a 45% decrease in inflation-adjusted income. 0.006% of the US population is trained to do this job. When do we say enough is enough?
