Noooooo!! This is so painful to wake up to. 😞 My initial thought is how amazing it would be for us all to simultaneously wear a red hat at the next meeting to celebrate him, and asking myself why we didn’t think about doing that for him when he was still with us. It would have had 20X the impact of a standing ovation for this hero who will not be forgotten.

Shoutout to @mpdrc for his incredible focus on patients. Direct quote from one of his pts: "From hands on experience there is no better doctor in the history of the world than Doctor Ruben Mesa. I had emergency surgery, and they wouldn't operate on me because first of all, you go in and you see a bunch of emergency room doctors and they don't believe the patient. I said, 'Look, I understand this. My blood counts are going to be scary. You guys, I get it.' They said, 'No, we're not going to operate on you until we talk to somebody.' Doctor Mesa was the only one who returned the call, right away returned the call, and none of the other doctors did." 👏 Dr. Mesa shows the way it should be. #patientcare @advocatehealth @AtriumHealth

So I see a #lymphoma pt urgently with a large mass in the liver on 10/30/25. We submitted to @AnthemBCBS for authorization for palliative #XRT. They took their time and on 11/12/25, requested a 3D vs IMRT plan. We submitted it, knowing the extra dosimetry work would be done for free. Today, we're told that it will take another 2 weeks for review because when #BCBS requests more information, that resets the calendar. This does not qualify for expedited review because-- 1. the patient can still breathe 2. the patient is not in the ICU and completely obstructed 3. the patient can still walk/talk So in essence, the patient has to be near death to where they can't get #radonc anyway. In the meantime, this tumor's not getting any smaller. #Healthcare in the #USA #outrageous #fixpriorauth @DutchRojas @BloodCancerUtd @AmericanCancer

If anyone knows @DRCamidge, they know how patient-focused he's been in his 20 years of leading lung cancer research. (And funny - just ask @JFreemanDaily 😉 .) But he hasn't been doctor only - he's been a stage 4 lung cancer patient, himself. Recently, he decided to open up about his experience after learning of his diagnosis 3 years ago. I had the privilege of hearing more about his experience firsthand and am grateful to him for continuing to help people, in spite of his incredibly busy schedule (BTW this was the 2nd time in 1 month he gave us @Patient_Story his time. Top 10 healthcare problems I hear from patients: -High cost of insurance premiums -High out of pocket costs even if you have insurance -Not sure whether insurance will cover or not -Delays and denials in care due to prior auth -Can’t reach the doctor easily -Can’t get appointments and tests done in time -Hospitals in small towns closing down (longer distances for care) -Doctors overworked and not able to spend time -Paperwork before, during, and after care -No peace of mind due to the above All of these are getting worse. Few of these issues can be solved by AI and stuff. What we need is real leadership and reform on the major issues: coverage, affordability, access, which if done right should not cost more than we already spend. There are many competing interests. Physicians have very little say in it. We spend so much and yet things are not ideal for our citizens which makes me sad.

I hear about this. This is not uncommon and it bothers me: Patient has cancer which is getting worse and causing pain Waits days for doctors appointment See the doctor. Scans ordered. But now waits days for an appointment slot to get the scans Scan is done and shows the cancer has worsened. Doctor prescribes new treatment Waits for days as insurance denies new treatment. Peer to peer call needed. Finally, treatment is approved. Waits for days again because chemotherapy appointments are backed up. Finally, the patient receives the treatment. But a month has passed. The cancer has progressed more. All of this with good private insurance. Imagine without. Oncology has advanced rapidly. Our medicines are very specialized. They require a lot of time, expertise, resources. Oncologists are stretched thin. It’s not the type of work that you can easily fill without compromising care. Many patients receive complex chemo and when there is a complication they end up without access to specialized care. We need to expand the oncology work force and limit the burnout that is causing good oncologists to leave patient care for other jobs or retire. There is no question we have made remarkable progress against cancer. And several promising new treatments are on the horizon. Will we have the resources to administer them safely and to all those who need it?

For years, I was told my family history of #lungcancer was “just a coincidence.” That multiple diagnoses on both sides of my family were only because they had a history of smoking, and that my own diagnosis at 39 - A 3rd generation on both sides of my family - was random. Research like this finally validates what patients like me have always known: family history matters. Screening guidelines need to reflect this reality - too many at risk people are being dismissed and missed. #WCLC25 #FamilyHistoryMatters #LCSM @IASLC

Me: Hi, I need to make an appointment with (Specialty) - just got the referral through from my doctor. Them: Okay great! Let's check the calendar..(10 seconds) Oh looks like they are all booked out for the REST OF THE YEAR. ...it's Early August, folks. Serious question: Has this ALWAYS been the case? I keep hearing (anecdotally) how hard it is to get an appointment, whether it's with PCP/internal medicine or a specialist. Just not clear if this was always the case and I didn't know because I didn't require such a long bench of healthcare providers till now 😞. #patientcare #patientcenteredcare #MedTwitter @Patient_Story

Was heading into an(other) appointment today when I saw an elderly woman lying on her back, she had clearly fallen and was in pain. A man was with her trying to get an ambulance. I went inside to see if I could get assistance for her (wheelchair? HCP?). When I went back out, I learned her name, Norma, and that she was taking an Uber to an appointment.. at 95 years old. I know there may be a million reasons why she had to get rideshare to her appointment. She may prefer it this way. I was equal parts admiring her independence and also feeling sad as a kneejerk reaction. Culturally, that would just be out of the question in other countries.

@artdmetz Same! I got an appointment with a PA first, then was able to get seen with a PCP. Otherwise I would have had to wait 6-8 months for a first appointment 🫠

@StephChuang This is a tragically extremely common and a consequence of misguided health care policy. Too many to list in a short tweet. Common for new M.D.s to graduate 300-400K in debt and primary care cannot generate enough income to pay that back and buy a house / start a family.
