HHS through @ASPRgov and @CDCgov is supporting @StateDept in the repatriation of 17 American citizens from the MV Hondius cruise ship affected by the Andes variant of hantavirus. All 17 are currently en route via @StateDept airlift to the United States, with two of the passengers travelling in the plane's biocontainment units out of an abundance of caution. One passenger currently has mild symptoms and another passenger tested mildly PCR positive for the Andes virus. As of now, the airlift will transport passengers to the ASPR Regional Emerging Special Pathogen Treatment Center (RESPTC) at the University of Nebraska Medical Center/Nebraska Medicine in Omaha, Nebraska before taking the passenger with mild symptoms to a second RESPTC at its final destination. Upon arrival at each facility, each individual will undergo clinical assessment and receive appropriate care and support based on their condition.

@qwerty12702 @quay_dr You could say that IF the quarantine period was over, but its not. More will likely test positive and the 1% difference in sequencing from the 2018 strain is significant as to unknown modes and severity of transmission in a populated environment.

Andes Hantavirus is terrible at human to human transmission.
So it is likely to phase itself out before reaching anything close to "pandemic" levels...

@Eesrepublic @IanCopeland5 Its not guaranteed, its only phased out in the past due to being most prevalent in rural villages. We have zero data to show what the result of transmission is in populated areas.

@HaroldJ_NEPA @outbreakupdates Correct, they will be assessed at the medical center in Omaha then allowed to quarantine at home. Not sure if the positive test and the symptomatic individual will choose to stay in Omaha in the quarantine unit. I guess we will see.

@outbreakupdates I hard two reports today: one, that the Americans (and a British national living in America) will be flown by military transport to the nation's top quarantine facility in Nebraska. The other, that they absolutely will not be quarantined.

@KrutikaKuppalli @hantavirus Its very possible. During Covid my son got sick, we quarantined properly for 14 days. The night before we returned to work and school I started showing symptoms, thus restarting the 14 day clock. 28 days total and I did it fine, but people are selfish.

I agree with this. The key question is whether individuals need to quarantine for @hantavirus at UNMC or whether they could safely complete quarantine at home with appropriate public health monitoring.
When I returned from Sierra Leone during the Ebola outbreak, I completed a 21-day home quarantine. That was challenging enough, and I can only imagine how much more difficult it would have been in a centralized quarantine facility.

If asymptomatic or presymptomatic transmission remains scientifically unresolved for Andes virus, what exactly prevents a quarantined passenger from exposing family members during 45 days of shared air, bathrooms, kitchens, and laundry?

Important: Andes virus transmission does not require “close contact” according to both prior studies and recent observations from the Hondius outbreak.

Earlier today I spoke with the doctor on the ship and he confirmed something. CDC messaging continues to say "prolonged, close contact" but that contradicts: 1) what's in the literature, and 2) what's happening on this ship. --> the doc told me a few who got infected/died DID have direct contact w/ very sick patient while treating them, but 3 who tested positive did NOT have direct physical or close contact - only shared time in a few spaces on the ship where people congregate (dining, lecture area)

The Hanta virus strain from the cruise ship has been sequenced, and it appears to be the known Andes strain (high mortality but not very infectious). It seems immoral to send passengers home via commercial flights during incubation period (would you want to be on that plane?)
