GPT-5.4 Pro continues to be the only model of its class. For anything really hard & complex, I throw it into the maw with every bit of context I can think of. More often than not, something very useful comes out. I can't get the same results from Codex or Code or anything else.

It’s not representative to look at the very next year. It took time for states to work through how they would administer the expansion and set up the process. It wasn’t overnight.

You could not, in fact, get coverage for costs above $5000 for $100 a month if you were one of the tens of millions of people who had any kind of pre-existing condition, or were just over the age of 50. And if you got pregnant, those plans offered limited coverage at best.
