The simufilam Phase 3 trials for #Alzheimers may have patients in earlier in the AD progression than expected - let me explain... Cassava Sciences, $sava, allowed patients that are 16 to 27 MMSE into the trial. 16 to 20 are considered Moderate and 21- 27 are considered Mild. To understand the points I am about to make you need to understand the analysis that $lly performed on their donanemab Trailblazer ALZ 2 trial. They accepted patients with MMSE between 20 and 28, but performed amyloid AND tau PET scans to confirm Alzheimer's disease in the brain. Plasma pTau181 was used for screening in the Phase 3 Cassava trials. Plasma pTau181 detects AD progression earlier that PET scans. In other words there could be patients that Trailblazer PET scan eliminated because they were earlier in disease, but pTau181 would catch. Something else that Trailblazer taught us was MMSE is not nearly as good at predicting disease state as PET pTau or Plasma pTau tests. The higher tau cohort performed significantly worse at 18 months with a 11.08 adas-cog13 decline vs 4.9 in the low and medium. For a 2 point difference in average MMSE, according to the ITO study of ADAS-Cog declines, you would only expect a 1 point different decline at 18 months. MMSE really should not be used alone to determine disease state. Trials should really use plasma pTau tests to screen. pTau is a much better predictor of true disease progression than MMSE. All of this has two impacts to the phase 3 trial of Cassava: 1. If we believe milds do better on simufilam, then the pTau181 scan will produces a milder patient base than one that is confirmed with PET. 2. The 16 to 27 MMSE allowance may still produce a more moderate base of patients, but when balanced with the earlier disease screening of pTau181, I think the overall population of RETHINK will decline ADAS-Cog12 at a slower rate than I originally thought. After this analysis, I think the ADAS-Cog12 placebo decline will be 4.25 or less. The trailblazer placebo for their total population declined at 4.45 at 12 months. The ADCS-ADL is the second primary endpoint for Cassava. Based on what patient families say, I feel comfortable about them out performing placebo. The issue I am finding is that there is not much available research for performance. One that I found found a population with MMSE of 21 (Refocus is 21.7) declined ADCS-ADL by 6.7 points [1]. So I am forecasting RETHINK placebo for change in ADAS-Cog12 of 4.45 or lower and change in ADCS-ADL of 6.7 or less. I believe the treatment arms will be near zero for both. A conservative estimate is they cut both declines in half, so 2.25 and 3.34. Those are my bets... what do you think they placebo and treatment performance will be? jamanetwork.com/journals/jaman…
