One of my favorite quick hemodynamic tools is the shock index: SI = HR/SBP Hypotension is a late finding and shock index detects early tachycardia and a softening of BP. Shock index has been most validated in trauma (looking for occult hemorrhage) but actually also predicts outcomes in pulmonary embolism, heart failure, and sepsis (although less precisely) On ShockCalcs, we've added a bunch of the evidence in the theory and practice section of the shock index calculator to check out 👇 Thresholds and interpretation in different populations also present on the page.

The best way to improve is not to focus on results. It’s to focus on habits. 27 studies: Behavior change goals are 3x more effective than outcome goals in boosting performance. Growth depends on altering the actions we can control. A key to progress is concentrating on process.

I often get asked if admitting to ICU for early pressors will clog up beds. My response: 💡 Admit early, stabilise with pressors, and they’re out in 24h once the antibiotics kick in. OR 💣 Wait two days, drown them in fluid, and admit them with multi-organ failure. Now they’re in for a week.
