@heniek_htw Blood quality, oxygenation and delivery depends on systemic vascular/endothelial homeostasis. Also systemic but somewhat heterogeneous are ECMs which along with vasculature support cells but ECMs undergo fibrotic stiffening with age.

@_K_Stiles It's possible now for AI to ask leading questions to determine student understanding. This would be part of the the educational ideal of individualized education. Learners would use AI to explicate more, areas of incomplete understanding and AI would never tire of repeating.

Hypertrophic cardiomyopathy (HCM) has long been framed as a “sarcomere disease” — MYH7, MYBPC3, TNNT2, TNNI3, and other rare pathogenic variants explaining a major fraction of familial cases. But this new medRxiv preprint argues that HCM is also a polygenic disease, especially in patients without identifiable sarcomere mutations. In a GWAS discovery cohort of 2,284 HCM cases and 4,525 controls, Lopes et al. identified three genome-wide significant loci: BAG3, FHOD3, and PPP1R3A, with PPP1R3A proposed as a novel HCM locus. They then meta-analysed their data with recent large HCM GWAS datasets, identifying 70 unique loci, including four additional novel loci: MYPN, YWHAE, NOS1AP, and OBSCN. The most clinically interesting part is the polygenic risk score (PRS). Using lead variants from the meta-analysis, the authors built an HCM PRS and tested it in 411,213 UK Biobank participants. The PRS was strongly associated with HCM diagnosis: HR 1.88 per SD increase, and HR 3.19 for the top 5% vs lower 95%. Even among people without diagnosed HCM, higher PRS correlated with subtle imaging traits: greater maximal LV wall thickness and higher LV ejection fraction. This suggests that common variants may shape the myocardial phenotype long before clinical disease is recognized. The key stratification: sarcomere-negative HCM. In 1,756 HCM cases with outcome data, sarcomere-negative patients in the top 20% of PRS had increased risk of sudden cardiac death or equivalent events: HR 2.72, 95% CI 1.03–7.17. This is important because sarcomere-negative HCM is often clinically frustrating: fewer clear Mendelian answers, more heterogeneous phenotype, and uncertain risk architecture. A PRS may help reclassify part of this “genotype-elusive” space as common-variant-driven myocardial disease. Biologically, the novel loci are plausible. MYPN and OBSCN encode sarcomere/Z-disc-associated proteins; NOS1AP links to electrophysiology/QT biology; YWHAE is connected to cardiac sodium-channel regulation. Caveats: this is a preprint, not peer-reviewed; cohorts were mainly European ancestry; and partial UK Biobank overlap may inflate PRS performance. Independent replication is essential. Still, the message is clear: HCM genetics is no longer simply “rare sarcomere variant: yes/no.” It is becoming a spectrum: monogenic, oligogenic, and polygenic myocardial hypertrophy.
