@StephenVLiu As a caregiver and Administrator for a large ALK support group for near 10 years, it is rare to not have TKI address the brain mets if the TKI addresses the mutation of the progression. We live longer so long term side effects is the main priority. Quality of Life Matters

One concern I have is radiation necrosis - a late toxicity seen more as survival improves. Uncommon (~10% in some series) but can be very challenging to manage, at times requiring prolonged steroids, off-label bevacizumab, even surgery. But SRS deferral not always appropriate...

@RamalingamMD @Humana From a patient perspective, results of cancer imaging should be provided same day as results are completed but especially if indicate progression. We seek doctors who understand this is information we NEED to know immediately and done via a phone call.

A pt of mine on Keytruda for NSCLC had restaging scans last week. Before I had a chance to meet the patient to discuss results, @Humana insurance sent a letter to him that he has progressed and will no longer be covered for keytruda. Has any of you encountered this? #LCSM .

@BrendonStilesMD Adding a few thoughts, surgeons are knowledged, most facing these decisions are not & rely on medicine to advise. Let's also note guidelines drive insurance approval and outside the box is often not even a choice unless you are wealthy
