Die große #Krankenhausreform unserer aktuellen Regierung stellt viele der etwa 1.900 somatischen Krankenhäuser in Deutschland vor große Herausforderungen. Eine der kontrovers diskutierten Leistungsgruppen im Leistungsbereich #Gefäßmedizin betrifft die Behandlung von Menschen mit Bauchaortenaneurysma: Etwa 490 Krankenhäuser haben im Jahr 2022 mehr als 12.000 überwiegend interventionelle (74%) Behandlungen in dieser Leistungsgruppe durchgeführt. Die mediane Anzahl betrug 18 Behandlungen. Im Referentenentwurf zum Krankenhausversorgungsverbesserungsgesetz (KHVVG) und in zahlreichen Vorträgen der verantwortlichen Regierungskommission ist von der Kappung an einer Perzentile die Rede, wobei die Kappung bei 80% der Fälle allgemein als Konsens mit moderaten Auswirkungen gilt. Anhand von Analysen der Qualitätsdaten der Krankenhäuser (2022) durch das Deutsche Institut für Gefäßmedizinische Gesundheitsforschung gGmbH (DIGG) der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin, Gesellschaft für operative, interventionelle und konservative Gefäßmedizin e.V. (DGG) konnte eine Behandlungsfallzahl in Höhe von n=25 für die Verhandlungen zur Vorhaltevergütung bestimmt werden. Hierdurch würde sich die Anzahl der an der Leistungsgruppe beteiligten Krankenhäuser auf 200 reduzieren, die im Median 41 Fälle pro Jahr und damit etwa 80% der bundesweiten Fälle behandeln würden. Diese Analyse stimmt mit den Simulationsmodellen der Regierungskommission überein, die in der Leistungsgruppe „Bauchaortenaneurysma“ mit Versorgungsdaten aus dem Jahr 2021 auf mindestens 22 Fälle an 214 Standorten kamen. Angesichts dieser nachvollziehbaren Methodik und der insgesamt bereits geringen Anzahl an Behandlungen gegenüber anderen Leistungsgruppen erscheint es nicht nachvollziehbar, warum die Politik in Nordrhein-Westfalen bei der Umsetzung der #Länderreform aktuell einen Grenzwert in Höhe von ca. 50 Behandlungen zu vorauszusetzen scheint. Dies ist weder medizinisch noch methodisch objektiv zu verstehen und gibt Anlass zu Kritik. Wir würden uns eine striktere und transparentere Einbeziehung der Fachgesellschaften und zuständigen Fachgremien wünschen, die sowohl den Einfluss auf die flächendeckende #Versorgungssicherheit als auch die Weiterbildung besser beurteilen können als Politiker/-innen und Referent/-innen. @VASCevidence
